HE WAS OPERATING BOLTING MACHINE IN #3 ENTRY. WHILE TRYING TO PUT BOLT IN DRILL HOLE, HE GOT HIS GOOT UNDER THE HEAD OF THE MACHINE. HE SAID HE NEEDED A LITTLE MORE CLEARANCE TO GET THE BOLT I N THE HOLE AND HE LET THE HEAD OF THE MACHINE DOWN ON HIS FOOT.
