WHILE INSPECTING THE ECAPE ROUTE FOUND A ROOF FALL AT THE INTERSECTION OF 34N + 44W. THE FALL WAS APPROX 32' X 28' AND 6-7' THICK. NO ONE WAS IN THE AREA AT THE TIME MSHA WAS NOTIFIED.

A MINER WITH 1 MONTH EXPERIENCE WAS DRILLING THE FIRST HOLE ON THE MINE ROOF FROM A NORMAL OPERATING POSITION. HIS RIGHT HAND WAS A THE CONTROL. WHEN A SIMPLE CHIP OF ROOF ROCK FROM THE HOLE F ELL AND STRUCK HIS RIGHT PUNKIE FINGER FRACTURING IT. HE WAS X RAYED AND SPHUTED AND RETURNED TO WORK.

TWOMECHANICS ASSIGNED TO ADJUST BUTTERFLY VALVE IN PIPELINE. MECHANICS REMOVED VALVE TO BE ABLE TO SEE OPERATION OF VALVE. EE HOLDING VALVE WHILE OTHER MECHANIC ACTIVATED VALVE. VALVE OPERATED AND ADJUSTED. EE ASKED FOR IT TO BE TRIED ONCE MORE. VALVE ACTIVATED AND DAMPER CAUGHT FINGERS OF LEFT HAND.
EE & CO-WORKER COMPLETED SCALING 79W ON & WERE LEAVING THE PILE. EE STEPPED ON A ROCK WHICH FLIPPED FROM UNDER HIS FOOT. EE FELL BACKWARDS ONTO THE MUCKPILE. THERE WAS NOT LT. ACCIDENT WAS REP ORTED AS A RECORDABLE DUE TO MEDICATION. EE REPOTED OFF ON 4/17 & 4/22 CLAIMED IT WAS DUE TO THIS FALL. BEING INVESTIGATED BY WORKERS COMP.

EE AND OTHER COMPLETED SCALING 79W & ON AND WERE LEAVING THE MUCKPILE. EE STEPPED ON A LARGE ROCK WHICH SLIPPED OUT FROM UNDER HIS FOOT CAUSING HIM TO FALL ONTO THE MUSCKPILE, LANDING ON HIS B ACK.
#1 CONVEYOR TIAL PULLEY WAS JAMMED WITH A ROCK. BELT WAS LOCKED OUT AND TAIL PULLEY GUARDING REMOVED. USING A STEEL BAR & HAMMER, EE BROKE UP THE ROCK. STORED TENSION IN THE CONVEYOR BELT, TUR NED THE TAIL PULLEY. EE'S LEFT HAND WAS PINCHED BETWEEN THE FRAME & THE STEEL BAR.

TWO MECHANIES AND A HELPER WERE MOVING A GEAR BOX AND HEADPULLEY ASSEMBLY INJTO A LOADER BUCKET. AS THE ONE MECHANIC STABLINED THE GEAR BOX THE OTHER TWO MEN WERE PUSHING THE ASSEBLY INTO THE BUCKET. THE GEAR BOX ROTATED ON THE SHAFT. THE MECHANIC TRIED TO CATCH IT. HIS ARM WAS PINCHED BETWEEN THE BUCKET AND GEAR BOX BROUED LEFT WRIST.
MECHANIC WORKIGN WITH HAND TOOLS DEVELOPED CARPUL TUNNEL SYNDROME IN BOTH HAND INJURED WAS OPERATED ON 1-21 FOR RIGHT HAND AND MARCH 10 94 FOR HIS LEFT HAND. NO LOST TIME. RESTRICTED DUTY.

CLEANING AROUND BELT CONVEYOR. NOTICED GUIDE ROLLER NOT TURNING. PICKING UP ANGLE IRON TO HIT ROLLER. CONVEYOR WAS RUNNING. ANGLE IRON CAUGHT BETWEEN BELT AND ROLLER WAS ROLLED FROM THE EMPLOY EES HAND STRIKING HIM IN THE FINGER.

INTERSECTION 50E38 WE HAD UNINTENTIONAL FALL OF ROOF. I FOUND UPON INVESTIGATION THAT SHALE IN AREA WAS THICKER. TOTAL OF 7 OF SHALE WAS ENCOUNTERED. THUS OUR 8 BOLTS WER NOT ANCHORING IN GYPSUM TO REMEDY THIS I HAVE DECIDED TO USE 10' BOLT FROM NOW ON & TEST DRILL EACH HEADING PRIOR TO BOLTING.
