Struck against stationary object

EMPLOYEE WAS USING A PICK TO REMOVE A GUARD AND TAIL PULLEY THAT WAS FROZEN IN THE MUD. HE HIT HIS LEFT ELBOW ON THE HOPPER BUT CONTINUED PICKING ON THE GUARD. HE THEN HIT HIS RIGHT INDEX FINGER ON THE GUARD. IT WAS BLEEDING SO HE WRAPPED IT IN A PAPER TOWEL. HE CAME IN AND PUT A BAND AID ON IT BECAUSE IT WAS BLEEDING. HE THEN WENT TO LUNCH AND RETURNED TO WORK AFTER LUNCH.

Struck by falling object

Electrican tighting up the guywire for overhead electric line and the guy wire insulator snap into, two parts and one part fell onto EE's lower left arm and cut the skin and left a deep cut.

Employee was pulling a rock out that was wedged between the primary crusher frame and some grease lines. When he dislodged the rock, it fell and pinched the little finger of his left hand against a steel cross member.

Right shoulder had been rubbing on seat in 2004 Cat 980G Loader. Employee reported on 11/28/07 that shoulder was swollen & sore and felt like a pinched nerve. Employee was sent to Occupational Health appointment on 11/29/07. Returned to work with restriction not to lift above shoulder. Was told to contact family Dr on high blood pressure.

Struck against stationary object

EE was jacking & cribbing the Cat 990 Loader to remove the rear end. Sliding wooden blocks under the loader he ran a splinted in palm of hand. He pulled exposed splinter out & continued to work the rest of the day(Friday). He came back Monday & his hand was swollen. He reported the accident & asked to go to a doctor to see if some of the splinter was still in the hand.

Fall from machine

EMPLOYEE WENT TO LOOK AT LINE BELT TO SEE IF REPAIRS WERE NEEDED. HE JUMPED OFF THE BELT ONTO THE GROUND. HIS KNEE HURT SOME AT THAT TIME. IT DIDN'T FEEL BAD UNTIL TUES. 7/5/05, WHEN IT STARTED TO SWELL. HIS LEFT KNEE SWELLED & HE COULD NOT HARDLY WALK ON IT.

EE WAS HELPING CLEAN SNOW FROM PRIMARY FINES BINHE WAS STANDING ON THE CATWALK CLEANING SNOW FROM INSIDE THE BIN. THE BELT CAUGHT HIS SHOVEL & WEDGED IN BETWEEN THE CATWALK ABOVE & THE BELT CAUSING IT TO SHOOT OUT & HIT HIM IN THE MOUTH DAMAGE & LOOSEN FRONT TEETH
