Loader operator noticed smoke coming from under feed hopper. Contacted plant operator. Plant operator called 911. Attempted to extinguish fire with fire extinguisher. Unsuccessful. Loader operator saw fire going up incline. Went to top of tower to extinguish with extinguisher. Unsuccessful and retreated to safe location.

EE was lifting up on a crank on the back of a trailer. The gear was stripped in a spot causing EE to fall forward and straining back.

The bucket on the loader caught the ground stopping the loader and throwing the employee into the windshield.

The crew was unrolling cable from a spool. The employee was still holding on to the cable when the spool moved backwards. The employee did not report any issue until 8/2/2013 when he claimed the cable jerked him and complained of a strain in his lower back. Employee was sent to the doctor on 8/2/2013 and the doctor recommened physical therapy, which was completed on 10/10/2013.
