While working on Plant 3, using a Porta Power Ram EE bent over to make repairs and felt EE's lower back spasm and tighten

Struck by flying object

While using a Chisel the metal tip broke off and landed in EE's left arm causing a cut/bleeding and metal in EE's arm

Employee was walking with a water hose spraying off a loader. Employee tripped on a rock and fell on the ground.

Using the skid-steer to clean up area around gravel crusher plant. EE stepped out and down from skid-steer and heard right knee pop. After working a few hours in pain, they went to clinic to be evaluated. Doctor thinks EE has a torn meniscus, waiting on MRI results to see when a possible return to work date will be.

Caught in, under or between a moving and a stationary object

IE and another EE were changing the roller on the sand stacker on Plant 1 they turned the roller on it's side and the weight of it rolled and pulled IE's gloved hand cutting/smashing EE's left pinky fingertip.

Employee was shoveling out the tunnel and tripped on an uneven surface, falling to their knees. Right knee hurt so they went to the doctor. Light duty restriction for one week and follow up on 07/18, scheduling MRI and still working light duty.

While working on a tail pulley (large, heavy, metal part) the pulley rolled off of a sawhorse and employee reached to catch it. Employee's back was hurt while catching/holding/lifting the part.

Employee was helping to lift a steel plate from a screen plant, EE turned and twisted right knee and it popped.

While doing routine maintenance on Plant 8, EE stepped off the railing and left knee popped. At the end of EE's shift EE went to the doctor and they said it was dislocated.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was welding on a metal tank (over a 2 day span) with a rubber & paint coating. While welding, EE inhaled toxic fumes and had trouble breathing & flu like symptoms. On 3/10/16 EE went to Evergreen clinic for testing.

While running plant 2 a rock got caught in the bin - ee pulled the rock out and another rock fell down smashing his hand. Went to the doctor on 7/15/13 x-rays - broken bone in tight hand - referred to surgeon appt Wed 7/17/13 can work with wrapped up hand.

While EE was hammering on a cylinder, that another IEE was holding, a piece of metal flew off getting lodged into EE's left forearm and wrist.

While doing routine maintenance on plant 3/crusher sand belts; they pulled a bolt from the motor causing it to fall and hit ee in the head and face.

Accident happened on an insolated section of the property (hill) being used on Sunday for a recreational event. All other property, plants and equipment was closed off to the public and competitors. The rider in the accident crashed his dirt bike during the competition.

Two ee's were repairing the grizzly. They were replacing a piece of iron railing which was strapped & chained to the loader. As co-worker operated the loader, IE was guiding him with hand signals. The iron bar hit a concrete wall on the plant & began swaying. He was hit by the bar twice, in the back and across the legs which caused him to be knocked to the ground, falling 3'.

EE was moving an oxygen tank with the JCB mine excavator bucket into the back of the welding truck and smashed his hand in between the bucket and the oxygen tank. He went to the local clinic and they determined his right hand was ok.

While getting out of the loader, EE slipped on the ladder and grabbed the hand rails to stop from falling and pulled or strained a muscle in his shoulder.
