WHILE ROTATING IMPELLER BAR TO NEW WEAR SURFACE BAR SLIPPED OFF OF PIVOT SLING CRUSHING EE FINGER. EE WAS USING HIS HAND TO GUIDE BAR PLACING HIS FINGERS AT RISK FOR CRUSHING INJURY SHOULD BAR SLIP OFF OF PIVOT. IN THE FURTURE, TEMPORARY HANDLES WILL BE WELDED ONTO BAR TO AVOID HAZARD EXPOSURE.
MECHANIC WAS USING A 6 FOOT STEPLADDER TO CLIMB OFF OF A ROCK TRUCK HE HAD BEEN WORKING ON. LADDER SLIPPED & TIPPED OVER. EE FELL ONTO LADDER STRIKING HIS BACK. EE WAS DIAGNOSED AS HAVING A CO NTUSION WHICH WAS CAUSING MUSCLE SPASMS. EE WAS PLACED ON RESTRICTED DUTY UNTIL JUNE 17TH.
