Alleged cumulative noise exposure.

Struck by falling object

Ice/mud buildup on outside of dump trailer fell off while trailer was elevated. Employee was walking by side of trailer and the ice/mud struck him on the top of the head. Employee not wearing hard hat. Small laceration resulted (required 3 staples to close) along with closed head injury without loss of consciousness.

CHANGING BEARING ON NDA UNDER CONVEYOR TAIL PULLEY WHEN COME-A-LONG BEING USED SUDDENLY LOST TENSION CAUSING EE TO FALL FORWARD, STRIKING HEAD ON SIDE OF CONVEYOR.

ON JUNE 20, 2002 EE COMPLETED HIS ANNUAL AUDIOGRAM IN WHICH A STANDARD THRESHOLD SHIFT AND RECORDABLE SHIFT WERE RECORDED. ETIOLOGY OF LOSS NOT YET DETERMINED. SPECIALIST SCHEDULED TO GIVE EVA LUATION AUGUST 13, 2002. BASED ON DOCTOR'S 9/5/02 EVALUATION REPORT, EE'S HEARING LOSS HAS BEEN DETERMINED TO BE NOT NOISE INDUCED AND DETERMINED TO BE DUE TO CHRONIC FLUID & EAR INFECTIONS.
