Walking down wet metal stairs and slipped causing right knee pain.

Miner was unloading a screen panel off of a forklift when the edge of the panel struck EE's left leg resulting in a laceration requiring stitches.

Employee was welding in a tight location and twisted EE's right knee.

Operator was cutting off a screen bolt with a torch and a piece of slag went into left ear burning a slight hole in the left ear drum causing the ear to become infected.

Employee was using a torch to repair a screen box and a piece of slag landed in EE's left ear causing a pinhole.

Employee was fabricating a mounting plate and a small metal particle got into EE's left eye. Employee went to the eye Dr and had particle removed. No lost work days, no restrictions.

Employee removed pin connecting conveyors, conveyor fell landing on employee's left foot breaking second metatarsal bone.

Employee walked under a conveyor struck his head and fell to ground landing on his right elbow

employee was scrapping off wet muddy material from a screen with a scraper when he strained his lower back

Employee was cutting out a worn wear plate on a conveyor and caught his right thumb on the 4"" grinding wheel causing a laceration to the thumb.

Employee was moving gravel screens when his right ring finger was pinched in the screens causing a laceration.

Employee was assisting in putting in a new 3/8"" tail pulley. The come along used in the process slipped causing the pulley/shaft to drop and pinch his finger.

EE was cutting a piece of belting with a utility knife, when it slipped causing a laceration to his inner thigh, that required several stitches.

Employee was repairing a truck when the leaf spring fell on his finger.

Employee was walking on stairs along pep screen. While learning over to clean off screen he slipped and hyperextended his right knee.

Employee claims he twisted his knee when he turned around to climb down the ladder of the 950 loader he was operating.

EMPLOYEE WAS ASSISTING WITH A CONVEYOR BELT REPAIR WITH A LOADER. BELT CLAMPS HAD BEEN PUT ON & CABLE COME ALONGS PULLED UP TO HOLD THE TWO ENDS TOGETHER. EE WAS ON THE CONVEYOR WHEN THE LOADER RELEASED THE PULL ON THE BELT & THE BELT ROLLED BACK PINNING EE'S LEG BETWEEN A ROLLER & THE BELT CLAMP.

Employee was operating a 950E loader and claims to have backed into a drainage culvert and jarred his back.

No specific incident - employee claiming soreness in shoulder- reported November 11 and stated the soreness started in July 2005- possible occupational exposure.

CARPAL TUNNEL

Employee was sliding beams back by the steel rack and smashed his fingers in between.

EMPLOYEE WAS USING A TOOL TO LIFT UP A PREP SCREEN AND TOOL SLIPPED. HIS FINGER WAS CAUGHT BETWEEN THE TOOL AND PREP SCREEN.

EE WAS ON TOP OF SCREENS SPRAYING ANTI-ICING AGENT FOR THE PRIMARY FEED. WHEN EE WAS COMING DOWN HE SLIPPED AND TWISTED HIS BACK.

EE STATES HE SLIPPED IN MUD AND HIT HIS SHOULDERON THE FORKLIFT.

EE WAS ASSISTING COWORKER IN EMPTYING BURMA BIN WHEN HE STEPPED OFF THE PLATFORM HE STEPPED ON A ROCK IN THE MUD AND TWISTED HIS LEFT ANKLE.

EE WAS USING A UTILITY KNIFE TO CUT OFF SOME BANDING. THE EE CUT HIMSELF WITH THE KNIFE.

EE WAS WALKING DOWN STEPS AND HEARD A SNAP IN HER LEG.

EMPLOYEE WAS WALKING DOWN LADDER OF LOADER TO SHUT THE FEED CONVEYOR OFF AND SLIPPED IN MUD TWISTING RIGHT KNEE AND FELL.

EE WAS STANDING ON A BANK BRINGING OUT WELDING LEADS OUT OF WELDING TRUCK. GROUND MATERIAL GAVE WAY UNDER EE CAUSING HIM TO FALL TWISTING HIS KNEE.

CLEANING ROCKS FROM CATWALK WEARING GLOVES, ROCK HIT EMPLOYEE HAND FROM ABOVE, SMASHING MY 5TH DIGIT ON RIGHT HAND.

EMPLOYEE WAS THROWING A CORD OVER EAST DRY PLANTSCREEN, CUT TWO FINGERS, TOOK THUMB NAIL OFF LEFT HAND FROM CORNER ON DRY PLANT SHACK.

EMPLOYEE POSITIONED WATER TRUCK TO BE FILLED ANDPROCEEDED TO FILL TANK. AFTER IT WAS FULL HE WENT TO SHUT OFF WATER VALVE AND TWISTED HIS ANKLE ON A ROCK FIXED IN THE GRAVEL AND FELL ON HIS LEFT SIDE.

EE HAS COMPLAINTS OF A SORE LEFT WRIST DUE TO OPERATING A LOADER.

EMPLOYEE WAS PULLING ON TAIL PULLEY WHEN HE TWISTED AND STRAINED HIS BACK.

EMPLOYEE ALLEGES DRIVING ON ROUGH TERRAIN CAUSED SORENESS IN BACK. COMPANY IS QUESTIONING THE COMPENSABILITY OF CLAIM.

PERFORMING TRUCK MAINTENANCE, TOOL FELL HITTING EMPLOYEE IN THE MOUTH. MEDICAL TREATMENT NOT ADMINISTERED UNTIL 8/15/00.

THE OPERATOR BACKED UP TO THE PILE WITH A BIN-TRUCK ON AN INCLINED AND BERMED RAMP TO DUMP HIS LOAD. WHEN HE RAISED THE BOX TO THE FULLY EXTENDED POSITION THE ENTIRE LOAD WOULD NOT RELEASE AND THE OPERATOR ATTEMPTED TO MOVE THE TRUCK FORWARD. THE TRUCK STARTED TO EASE OVER ONTO ITS PASSENGER SIDE WITH THE BOX COMING TO REST ON THE BERM. INJURIES CONSISTED OF MINOR BRUISES CUTS SCRA
