Employee was operating a 950E loader and claims to have backed into a drainage culvert and jarred his back.
No specific incident - employee claiming soreness in shoulder- reported November 11 and stated the soreness started in July 2005- possible occupational exposure.

BLASTER RECEI VED SEVERE LACERATIONS TO FACE, BROKEN NOSE, BROKEN FEMUR, BALL & JOINT CONNECTING FOOT, TWO FRONT UPPER TEETH KNOCKED OUT AND HIS BODY WAS EMBEDDED WITH SAND.
