Employee pulled on fuel hose after fueling the dozer and injured EE's shoulder.

Employee was walking the mud in the pit after lots of rain and twisted right ankle on a rock. EE's ankle is sprained.

Absorption of radiations, caustics, toxic and noxious substances

Employee was raising stacker when a hydraulic line broke spraying hydraulic fluid into the employee's face and eyes. Employee sought medical treatment at local hospital. Eyes were flushed and employee was released. Employee has a follow-up appointment with employee's regular eye doctor on 9/14/18.

Employee was pulling blocking from jaw during crusher tear down and felt a pain in low back. Employee sought treatment. Low back muscle strain with 20lb weight restriction and muscle relaxer.
