Portable LJ 54 Cone Metal/Non-Metal

Employee jammed their left pinky finger between the door and door handle when locking up the job trailer.

Employee was moving a 55 gallon drum next to another drum and pinched 2 fingers (left hand) between the barrels. Employee sought medical treatment at a local hospital. Both fingers were x-rayed, neither is broken. Left pinky finger was lacerated: 2 stitches.

Employee climbed up on the conveyor belt during setup to get to an electrical box. It had rained the night before so employee's boots were wet from the puddles on the ground. Employee's foot slipped causing employee to fall off the ledge they were standing on.

Employee caught arm on door latch of loader causing a laceration.

Employee stepped off ladder, lost his balance and stumbled to ground landing on hands and knees.

Employee was dismounting from dozer when he stepped into divit on ground and twisted his left ankle.

Employee was climbing off of a dozer when he twisted his knee.

EE WAS WORKING AROUND THE CRUSHER GIRZZLY WHEN A ROCK FELL OFF THE GRIZZLY AND ON THE EE'S HEAD AND SHOULDER. EE BEGAN TO EXPERIENCE NECK PAIN AND ARM NUMBNESS, WHICH SUBSEQUENTLY WORSENED. TH E EE SOUGHT MEDCAL ATTENTION ON OCT 31ST AT WHICH TIME, THE INJURY BECAME A REPORTABLE LOST TIME ACCIDENT.

WAS OPERATING LOADER AT PIT SITE, WAS CLIMBING DOWN TO GROUND, STEPPED WRONG AND HURT FOOT. TORN LIGAMENTS ON TOP OF LEFT FOOT. EE RETURNED TO WORK FOR 3 HOURS ON 10-16 AFTER INJURY. HE DID NO T WORK FORM 10/17-10/20. HE IS BACK TO WORK FULL CAPACITY ON 10/23/00.
