The operator was exiting the skid loader, EE's foot caught the chain hook on the top of the skid loader bucket causing EE to fall.

Employee was moving a 55 gallon drum next to another drum and pinched 2 fingers (left hand) between the barrels. Employee sought medical treatment at a local hospital. Both fingers were x-rayed, neither is broken. Left pinky finger was lacerated: 2 stitches.

Employee climbed up on the conveyor belt during setup to get to an electrical box. It had rained the night before so employee's boots were wet from the puddles on the ground. Employee's foot slipped causing employee to fall off the ledge they were standing on.

TH E EE SOUGHT MEDCAL ATTENTION ON OCT 31ST AT WHICH TIME, THE INJURY BECAME A REPORTABLE LOST TIME ACCIDENT.
