Employee had replaced a fuel filter on a dozer during routine preventative maintenance. During this process, ee gloves became wet with diesel fuel. When ee attempted to stand up, ee hands slipped off of the hand rails. This caused ee to fall backwards and land on the left side of ee body, and the left side of ee head.

Not paying attention when departing truck/vehicle that was parked and not running. Missed last step and right ankle twisted.

Employee was turning a nut with a wrench while performing routine maintenance on shaker. Employee slipped during this process and fell to the walkway causing lower back pain.
