AS THE PIPE FELL IT STRUCK HIS FOUR LITTLE TOES, MASHING AND OR SHEARING THEM OFF. THE PIPE HAS ALWAYS TAKEN EXTREME FORCE TO REMOVE. THIS TIME IT SLIPPED FREE EASILY ADN UNEXPECTLY.

EMPLOYEE WAS TIGHTENING A CHAIN CLAMP FROM ABOAT. AN "EAR" ON THE CLAMP BROKE OFF CAUSING THE PIPE WRENCH TO SLIP. WHEN THE WRENCH SLIPPED HE LOST HIS BALANCE AND FELL AGAINST THE SIDE OF TH E BOAT BRUISING HIS LEFT SIDE "RIBS".
