Magnolia Frac Sand LLC · Struck against stationary object

Forklift used to lift a pallet of repair parts to an upper level. Employee was on the ground & when ee walked around the forklift, ee hit the lift tine. EE cracked a tooth on upper left side. EE was taken to the dentist for evaluation/treatment; a temp repair was made. The first repair did not hold. The employee is pending treatment for installation of a crown.

Doing plant maintenance/repairs, eye protection (face shield and glasses) were worn during the work process. Wiped face/eyes afterward with glove/hand - eye became irritated. Flush improved, but did not completely relieve.
