Employee slipped on the mud and fell while rolling out a 6in pump hose and strained their right shoulder 4/8/21. Reported it to manager 4/9/21 and refused medical evaluation until 4/28/21 at 1015am. EE was prescribed medication 4/28/21 making this a recordable injury.

Employee slipped getting something out of bed of truck and fell against a gear that was in the bed of the truck. Impact broke skin.
