Struck against stationary object

Employee was dismounting an excavator and placed right foot on a shale rock at the same time ee left foot came off the bottom step when the rock rolled. When the rock rolled ee lower right leg broke.

Employee exiting machine after trouble shooting; missed step on machine; landed on left leg / foot and sprained left knee

Struck against stationary object

Employee was exiting a 10 wheel haul truck when the door handle broke off in his hand when he pushed down on the handle. The sharp edge on piece that was still attached to the door handle mechanism cut his hand. The cut required stitches to close the wound.

Employee was removing rock from the side door of the crusher. A loose rock rolled down and smashed his finger against the side door. He received a laceration that required three stitches and also broke the tip of his finger.

Employee was hitting the side of a bin to dislodge material. The sledge hammer deflected off the side of the bin and struck him on the right knee. A lineal fracture occured to the knee.

Fall to the walkway or working surface

Employee was performing repairs on a stockpile truck. As he went to his maintenance truck to get some tools, he slipped on ice and fell on his back. The employee experienced a strained back as a result of the fall.

REPAIRS WERE BEING MADE TO THE RIGHT FRONT WHEEL. THE WHEEL COVER DROPPED OFF AND ROLLED ON THE GROUND TOWARDS INJURED EE. AS THE COVER ROLLED BETWEEN HIS LEGS HE TRIED TO KEEP IT FROM HITTING HIM. IN DOING SO HIS FINGER ON RIGHT HAND GOT CAUGHT IN THE COVER AND BROKE HIS FINGER.

EMPLOYEE HAD LOOSENED A TAIL PULLEY FOR REPAIR. HE HAD HIS FINGER BETWEEN THE FRAME & ADJUSTING NUT WHEN BEARING SLID FORWARD & CAUGHT HIS RIGHTINDEX FINGER BETWEEN THE NUT & THE FRAME TAKING A GASH OUT OF HIS RIGHT INDEX FINGER REQUIRING SOME SKIN GRAFTING.

EMPLOYEE NOTICED DOOR ON PASSENGER SIDE OF HIS TRUCK WAS NOT SHUT ALL THE WAY, HE THEN REACHED CROSS AND OPENED THE DOOR TO SHUT IT AND SHUT HIS LEFT THUMB IN THE DOOR SMASHING IT AND CUTTING IT. THE CUT REQUIRED EIGHT STITCHES.
