Mid MO Mining LLC · Caught in, under or between a moving and a stationary object

Employee was checking fluids on a haul truck. EE was in between the door and haul truck frame when another employee got into a skid steer and backed into the open hood on the haul truck pinning EE in between. Upon evaluation there were no visible cuts or lacerations, the supervisor told EE that EE needed to get medical attention, which EE refused at that time.

Trees and brush were being cleared on mine property and burned outdoors. The wind blew a spark to an adjacent location and caught some leaves on fire. Employees immediately got fire extinguishers to extinguish the fire. EE inhaled some smoke that caused some breathing difficulty.

Because of poor illumination the miner could not see and did not use his flashlight. He used his cigarette lighter and the fuel barrel exploded, causing the miner to be burned. He was taken to a hospital with a trauma center for treatment of burns to his face, arm and hand.

Fire started on two trucks parked outside of our Office/Shop Facility and spread to Office/Shop and burned the facility down. Both trucks burned along with another truck parked inside of the Office/Shop. The Office/Shop Facility was a total loss.
