A fall of ground occurred in ME-163V crosscut. A mechanic was parked in the crosscut with pickup TP-74 when the fall of ground occurred, resulting in a fatality. After the fall of ground covered the TP-74, the vehicle caught fire, promting evacuation of the mine.

Employee was lifting a 10" aluminum irrigation pipe onto a trailer when a pipe ee was lifting hooked glove causing it to pull hand toward another piece of pipe on the trailer, smashing finger. Employee was immediately taken to the local hospital.

While lifting a starter from engine block the employee felt a stabbing pain or pop in his right elbow.
EE was pulling water hose in mine. Hose caused twisting because of tension on hose. Weight was on left foot which was on a loose rock. EE lost control of his footing and spun 180 degrees and fell to ground. No equipment was involved. EE suffered fracture to bone in left leg and misaligned left ankle.
Powderman was loading CB55F xc when he asked his partner to retrieve a booster from #73 powder buggy. While the partner was retrieving the booster the powderman waited in man-basket. During this time a rock fell off rib striking the powderman resulting in cuts to his head/hand requiring stitches. The heading was scaled by the powder crew prior to loading the round.
EE allegedly claims head, shoulder, back injury when he was unloading by hand and moving a piece of steel weighing 78 lbs. He stated he caught his left heel causing him to trip and fall resulting in the injuries.
EE allegedly claims right shoulder injury from losing control of a fuel hose on Lube Truck #77 on 7/23/12. He stated the hose jerked out of his hand when he was cleaning it. EE did not report injury until 9/14/12. Workers' Comp denied claim on 9/26/12.
3:35pm it was determined ground had fallen above the anchorage zone of bolts located at A64O xc. 3:48pm the condition was reported to MSHA.A squared up round was shot consisting of a right rib slab and floor. Previous inspections of the area did not show reason to install additional ground support beyond our normal bolting plan.
EE was priming fuel system on #23 bolter and requested a jump start from TR13 lube buggy. Lube buggy operator set brake and chocked the tire then reached in and flipped the engine to high idle. Lube buggy was left in gear and overcame park brake proceeded forward and pinned EE against the bolter. EE's left leg was pinned below the knee causing muscle damage. 2 EE's affected.
EE was operating TR13 Lube Buggy. Pulled up to #23 to jump start. Set brake and chocked tire. Reached in to set high idle on Lube Buggy. Lube Buggy was left in 2nd gear. TR13 overcame brake and proceeded forward throwing EE to the ground. Resulted in EE injuring left knee. Two EE's affected.
EE was assisting in the operation of core drilling. The top of the overshot reached the guide wheel and shot forward hitting the EE in the face. EE received stitches to the head. There was no lost time from work.

EE was loading a face when rock came off of face and struck EE in head and middle of back. EE has been hospitalized. Extent of injuries unknown at this time.
EE was walking on snow packed ice from coag distribution site. Stepped over white pipe then started over black pipe when his left leg slipped out from underneath him. Injury - right knee acl tear.
EE was scaling a rock that was approximately 20 feet above ground. A co-worker was also scaling and loosened a rock that caused another rock to break loose and fall hitting the injured EE in the left shoulder.
EE claims lower back strain due to driving #49 haul truck in mine. On 7/30/10, physican diagnosed lumbar strain with muscle spasm.
EE alleges groin strain due to operation of haul truck underground. Physician diagnosed on 6/23/10 "no work until released due to medical condition."
EE fractured right wrist while installing bead ring on tire. Ring slipped and caused tire handle to jamb elbow resulting in injury to wrist.
EE fractured wrist when latching mechanism on a multiplier gave way, swung around under pressure and struck EE on left wrist.
While mucking in face the loader bucket caught on face and bounced the loader jarring his neck.
EE complained of right arm and neck pain after lifting a piece of wire mesh from his tractor. EE went to the hospital and no diagnosis has been determined at this time by physician. EE must await doctor to determine injury prior to returning to work EE missed 1st scheduled shift on 4/17/10.
On 2/25/10, it was reported at 9 pm to the Libby Rural Fire Department, that the Stimpson Plywood building also know as Kootenai Business Park Industrial Center located at 990 E 5th Street Extension Libby, MT 59923 was on fire. Genesis Inc. stores railcars of concentrate at the facility. These railcar were in the building waiting shipment from the Port Authorities to the smelter.
(Not on mine property) Property owned by Kootenai Business Park Industrial District in Libby, MT. Fire started in collapsed section of building where demolition work was being done. Fire department did not try to extinguish fire and it spread to railcar load area.
EE was cleaning parts and got some solvent into a cut on his left palm. EE was treated for an infection in his palm.

Employee was climbing down the ladder of a 980 G loader. When he slipped and fell to the right on to the rear tire thru to the concrete floor in the surface shop.
Employee cut top left index finger with utility knife while cutting a rubber hose. Employee received stitches.
EE twisted ankle and broke small bone on top of left foot while walking underground.
Employee was pinned between flatbed pickup and bit bin dumpster. Employee suffered two broken legs.
Employee was scaling out of a JLG basket. Employee scaled a large piece of rock down on to boom of the JLG, causing the boom and basket to rock down causing employee to fall out of basket. Employee struck boom with his ribs stopping with fall protection. Employee suffered cracked ribs.

Employee slipped and fell flat on back. No treatment until 5/8/09. Injury determined, was torn muscle in shoulder, surgery required.
Left truck in gear and got out of truck to lower jacks when truck moved and hit him.
While trying to find a leak in a hydraulic hose, the hydraulic system surged and sprayed and cut the employee's finger.
EE alleges he slipped while walking down the conveyor and twisted his knee. Accident was not reported until 10/10/08 and was not accepted until 11/11/08. 10/10/08, EE missed 1st scheduled shift.
EE attempting to sit in his chair in the primay crusher and fell backwards. EE didn't go to a physician until 8/29/08. Was not aware of his results until 9/4/08. EE received a precription for pain in leg/back.
Injury slipped and fell on a rock injuring some muscles and possilby bruising his kidneys. He has had no surgery, but is being treated by physical therapy. He drew his first social secuirty check, also in July, so he is retired and will not return to work for us.
Employee was changing powder totes at the powder magazine on the powder truck. Employee stepped on the transmission and slipped on some hydraulic oil. Employee tried to catch himself with his left arm and it bent his arm back far enough to pop it out of the socket. Employee lost zero days and returned for his next scheduled shift.
Employee was troubleshooting electrical control problem in an electrical box. He bumped support for the box lid with his elbow. As the lid came down and he moved out of the way, he ran the back of his hand over a sharp edge. Resulted in 11 stitches on the back of his right hand. No lost days - finished his shift.

A fall of ground occurred in LW59C inby section. The fall of ground was approx 40'long X 30'wide X 2'-10' in depth. The fall of ground occurred some time after 10pm on 12/29/07 and was found mid morning on 12/30/07. It was inspected by safety engineer and mine manager. Was determined reportable at 11:45am.
EE was near the intersection of ME-163V crosscut when the fall of ground occurred. As EE was running away, he fell and injured his left knee. EE was taken to the emergency room.

Miner was removing drill steel from jumbo boom. Drills steel sprung and pinched right thumb btween drill steel and feed beam on jumbo. He received small fracture to thumb between upper. Knucket and lower joint. He finished shift and missed zero days.
While removing a ramp in the EOB-A drive a portion of the right rib failed and began to break out under the supported ribs above. These were pulled out past point anchor on 3/23/07. On 3/23/07, we determined that some boltz fellout of the rib above. The right rib went in approx 15' h x 15'w x 10-15' into the rib.

A fall of ground occurred in UE 159 after blasting the heading. The ground fall fell approximately 15 ft back from the standing face and went up in the back approx 10-12 feet high.
Repairing links on Machine with grinder. Metal got past safety glasses into eye. Removed by Doctor.
On 7/24/06, EE was working on a driveshaft while removing the drive shaft He was struck by a pry bar in the forehead. He recieved 7 stitches in his forehead. No degree lost.
A fall of ground occurred between shifts on the morning of 7/23/06. The fall of ground occurred in the south adit while rehabing old wooden timber sets. Approx 500 cubic yards of material came down when old timers failed.
Employee strained lower back while walking down ME157R ramp. Employee slipped and caught himself, causing a strain in his back. Injury was not reported until 7/17/06.
Ball mill operator was working in the 8th crossover cleaning. When cleaning, she hooked her foot in a rolled up hose. She lost her balance and tripped forward landing on her left wrist. Her left wrist was fractured. A splint was applied on 6/20/06 and she returned to work on her next scheduled shift.
On 5/2/06 a fall of ground occured in ue-162m intersection the ground fall occured after shooting ue162 worth and continued falling on to 8 M back couple of houses it approx arched up about ten feet, going to feet past point anchor.
ON 3/15/06, EMPLOYEE WAS STRUCK BY A TIRE FALLING OFF OF A FLATBED TRUCK. THE TIRE ROLLED OVER LEGS AND TIPPED OVER. EE FINISHED HIS SHIFT & TWO MORE SHIFTS WERE WORKED. EE WENT TO THE DR. & WAS TOLD TO TAKE 3 DAYS OFF. EE QUIT ON 3/22/06 & DID NOT WORK THAT DAY.
A FALL OF GROUND OCCURED IN UE160 SOUTH DRIVE. A SECTION OF BACK CAME OUT APPROX. 20'WIDE X 30'LONG X 10-12'THICK. THE AREA WAS FLAGGED OFF & CLOSED THE HEADING TO FURTHER EXAMINE THE GROUND FALL.
EE was cleaning under #3 belt return idler, while bending the guard up to clean, the guard broke and struck EE in the mouth, hitting his top front teeth. EE went to dentist and received no treatment. Dentist said his teeth might fall out later. EE went back to the dentist on 4/10/06 to remove the 4 front teeth. EE received treatment on 4/10/06.
Employee was walking down 10 belt in the secondary crusher. She slipped and grabbed onto both handrails & cracked her sternum. She didn't go to the doctor until 3/3/06. I became aware of this lost time accident on 3/6/06.
I BECAME AWARE OF A LOST TIME ACCIDENT ON 2/18/06, WHEN EE DID NOT SHOW UP FOR HIS REGULAR SCHEDULED SHIFT. EE (A LUBE MAN) INJURED HIS LEFT SHOULDER WHILE PULLING OUT A HOSE REEL ON HIS LUBE TRACTOR. EXTENT OF HIS INJURY IS UNKNOWN AT THIS TIME.
I BECAME AWARE OF A LOST TIME ACCIDENT ON 2/14/06. EE WAS LOADING A TOP SLICE ROUND WHEN HE SLIPPED AND FELL ON A ROCK (WET ROCKS ON MINE FLOOR), HITTING HIS KNEE. EE FINISHED HIS SHIFT, BUT DID NOT RETURN FOR 2 DAYS. HE THEN WENT TO THE DOCTOR AND GOT A RELEASE TO WORK ON 2/18/06. HE WORKED ONE SHIFT AND DID NOT RETURN FOR THE REST OF THE WEEK.
A fall of ground occurred between EB76-EB77 bench bottom and top slice. This area was an inactive area. The ground fall did go past point anchor on resin bolts. Area is flagged off and barricaded from access. The time of fall is not sure; it happened at shift change.

Employee was walking to the bus to start shift when he fell on an ice patch. He twisted his knee and went to the doctor.
EE was bench drilling near a haul. While drilling a rock about 4 inches by 6 inches fell from the back and grazed his left arm. He looked up and then another rock about 6 inch x 6 inch came and fell on is quardracep muscle in his leg. He was treated from bruised muscle, he was to put ice and elevate.
EMPLOYEE WAS ATTEMPTING TO LIGHT THE PROPANE HEATERS, WHEN A RELIEF VALVE POPPED AND WAS RELEASING PROPANE. HE STARTED TO LEAVE THE AREA WHEN THE PILOT LIGHT CAME ON AND IGNITED THE PROPANE. THIS CAUSED A SUDDEN FLASH OF FIRE, SINGEING THE EMPLOYEE'S HAIR AND FACE, CAUSING 1ST AND SOME 2ND DEGREE BURNS ON HIS FACE.
Employee failed to move his hand away from the drill rod when lowering the core barrel/overshot assembly into the drill rod. The overshot pinched his hand on the drill rod. Laceration on right hand on webbing between thumb and index finger resulting in 7 stitches. No permanent damage.

A fall of ground occured in W-23 area of the mine. This area was an inactive area of the mine. Approx area of ground fall was 90' by 120'. Two pillars failed causing the ground fall. No injury occured.
EE was treated for a bilatered hernia from severor coughing while at work state find notified the 50 parts and hir dept. on 8-7-05.
EE was placing shot crete in the hopper of the shot crete machine, while taking a break, he placed his hand on the cement truck shoot. The cement truck driver was adding a section to shoot. Other ee fliped the shoot down pinching ee left hand between the joints of the shoot. He was taken to the dr, 2 broken fingers.
Rod fell out of the rod handler, pinching ee's finger between rod and chain of #15 bench drill. First aid was administered. On Monday 5-2-05 he went to the doctor and found to have two broken fingers.
Fall of ground above the anchorage zone, eastern body bench 79-80, 20' wide by 120' long and 12' to 15' in height. No injury occurred. 980 loader was slabbed hitting bucket.
EE WAS IN BOLTER BASKET, BARRING DOWN ROCK, WHEN ROCK CAME DOWN AND PINCHED EE'S FINGER BETWEEN THE SCALING BAR AND THE BOLTER BASKET. (FLESH AND MUSCLE PULLED OFF BONE; SURGERY TO REATTACH WAS SUCCESSFUL.)
EE WAS STEPPING OFF GETMAN TO MOVE OUT OF THE WAY. WHILE STEPPING OFF, EE ROLLED HIS ANKLE.
