TIGHTENING DOWN A NUT WITH A CRESENT WRENCH, THE WRENCH SLIPPED OFF THE NUT RESULTING IN A CUT TO HAND. EMPLOYEE SOUGHT MEDICAL CARE: RECEIVED 5 STITCHES AND ANTIBIOTICS.

EMPLOYEE WAS INTERMITTENTLY EXPOSED TO SULFUR DIOXIDE DUE TO FLUCTUATON IN WIND DIRECTION AND SPEED. EMPLOYEE REMOVED RESPIRATOR INTERMITTENTLY TO TALK DURING START-UP TRIALS OF ROASTER.

Over exposure to u.v. rays. A 100 watt mercury vapor flood light outer shell broke and inner shell continued to burn. All employees in the area received burns as well as eye irritation.

ON ROLLING STAIRS AT SHOP. STAIRS MAY NOT HAVE BEEN LOCKED IN. STAIRS MOVED. FELL DOWN 11 STAIRS (ROLLING). ENDED AT BOTTOM ON BACK.
