Struck by flying object

In the late morning on 8/28/08, a small speck of metal got into EE's right eye. By 7 PM his right eye was very irritated and swollen. He went to ER and received medical attenion (metal sliver removed). No damage to eye. EE returned to work the following Monday, 9/1/08.

Over-exertion in lifting objects

""I lifted a rock and pulled a muscle in my left knee. It felt like a cramp. I thought it would work itself out but instead the pain began to travel up into my back as the day ended. When the injury incurred, I told 2 co-workers. On July 28, 2008 I saw Doctor. He checked for inflamed muscle and took me off work.""

EE suffered a left middle finger injury. Went to DR on 7/25/08, diagnosed a fracture finger.

Pulled back muscle.

Finger injured when rock popped back after going thru chopper/splitter. Reported to supervisor, Medical treatment sought immediately, medical treatment. **No sutures, glued to minimize scarring.**

EE was moving material around.EE picked up a piece of scrap metal abd while walking he slipped on some ice and twisted his back, but did not fall.EE told coworker he hurt himself, but did not feel that it was serious. Next day felt sore and pain, went to Dr and was informed he has piched nerve.

EE was stacking a pallet of rock when he was hit with a rock in the lower calf. When he got up he could not detect where the rock had come from. He told another EE & later his supervisor. He tried to walk it off but couldn't. Went to ER @ 2:30pm & was given CT Scan & Ultrasound, pain & swelling medication & told to Ice & Elevate leg. MRI scheduled & told not to work until cleared.

Struck against a moving object

I was operating the fork lift. Going backwards I was looking over my right shoulder. The forklift went over a large rock and bounced and jarred my back. The pain was terrible when it first happened. I let the company know about it later that day. I saw Doctor 9/5/07. He gave me muscle relaxers and said if it is not better in 7 days to call and make an appointment.
