Newmont Corporation: 18 recorded fatalities controller

A miner was fatally injured when the miner was run over by the LHD loader the miner was operating underground.

The employee was fatally injured when he was travelling on a shaft conveyance and came in contact with the shaft furnishings.

Employee was in the procees of building a berm when his loader went into the stope.

A fatal accident occurred at the Exodus underground mine on Friday August 31st at approximately 13:30. The employee was preparing the top cut of the 4410-315 long hole stope, when he fell into the stope.

While performing a section repair in sidewall of a large earthmover tire the associate was discovered unconscious and unresponsive inside the tire by a mining company worker.

Surface Mine Engineering Sampler was backing up his flatbed pickup on a a drill pattern. A Drill Operator had dismounted his drill and was walking on the pattern. The drill operator walked behind the pickup and the sampler was not aware the the driller was there. The driller stopped walking and was run over by the flatbed pickup, resulting in fatal injuries.

Two employees (welders) were dismantling a large tool rack. The welders were cutting 8'X4"X1/4" plate steel from the frame. The rack became unstable and tipped over on top of the employees. One employee sustained fatal injuries and the other received injuries to his back and legs.

The operator of a 2 yard underground loader received fatal injuries when the loader he was operating fell into a hole. Operator was confirmed deceased 07/02/2007.

EMPLOYEE WAS CONDUCTING TESTS ON HAUL TRUCK 085. WHILE THE TEST WAS BEING CONDUCTED, THE TRUCK ROLLED OVER THE CHOCK BLOCKS AND STRUCK THE DOORS OF THE SHOP. THE DOORS FELL AND STRUCK THE EMPL OYEE CAUSING FATAL INJURIES.

A FATAL ACCIDENT OCCURRED AT THE 5520-237 LONGHOLE STOPE WHEN A WATER TRUCK OPERATOR BACKED OVER THE BERM AND INTO THE STOP WHICH AT THE TIME WAS BEING BACKFILLED.

TWO EMPLOYEES WERE SERIOUSLY INJURED ONE FATALY WHILE UNHOOKING A SLING FROM A CONVEYOR SECTION MOVED INTO PLACE BY A 235C BACKHOE(CAT) TWO TEMPOARARY DRILL STEEL LEGS HAD BEEN WELDED ONTO THE CONVEYOR EE FOR SUPPORT ONE LEG FAILED CAUSING THE CONVEYOR EE TO ROLL RT THEN FALL TO THE GROUND PINNING BOTH MEN BENEATH IT SECOND PERSON SERIOUSLY INJURED WITH LOSS OF ONE LEG TO DATE

13017PROX 1010 THE GYRO CRUSHER OPERATOR OBSERVED EMP LAYING FACE DOWN AT THE BOTTOM DECK OF THE CRUSHER. INSPITE OF EFFORTS ON BEHALF OF MINE ER PERSON, HE WAS UNABLE TO BE REVIVED PRONOUN 35404EAD AT HOSPITAL. PER FAILED TO REVEAL CAUSE OF DEATH

CONTR USING 20 TON CRANE TO MOVE LARGE PIPE.EMPL WORKING AS SPOTTER WITH HAND ON PIPE TO GUIDE IT WHEN BOOM OF CRANE CAME INTO CONTACT WITH 60,000 VOLT POWER LINE.

EMPL DRIVING EMPTY TRUCK BACK TO SHOVEL FOR A LOAD,APPARENTLY WENT TO SLEEP & TRUCK LEFT THE ROADWAY,CLIMBED THE BERM THEN FELL DOWN 45' TO NEXT BENCH LANDING ON LEFT SIDE ROLLING ONTO THE TOP .DRIVER FELL FROM CAB AND WAS CRUSHED BY ROLLING ROCKS.

WHILE LOADING TRAINS,EMPL LEFT DOOR ON RAISE OPEN,CAUSING SPILL OF MUCK AND WAS BURIED.EMPL PRONOUNCED DEAD ON ARRIVAL,SUFFOCATION.

A 50TON WATER TRUCK 10,000 GALLON CAPACITY WAS BEING FILLED AT A WATER STAND, WHEN DUMP BRAKE FAILED AND ALLOWED IT TO RUN FREE ONTO #8 ROAD AND STRUCK PICKUP #105 HEADON, WHICH WAS OPERATED B Y THE VICTIM.

EMPLOYEE APPARENTLY HAD HIS BELT AND CLOTHING TO BECOME ENTANGLED ON SHAFT CAUSING CONSTRUCTION TO CENTER OF BODY RESULTING IN DEATGH

TRUCK FELL OVER EDGE OF WASTE DUMP AND ROLLED. DRIVER FATALLY INJURED.
