Haul truck driver closed the truck door on their own finger.

100 ton haul truck was preparing to dump when the ground gave way causing the truck to flip over backwards.

While driving the front end loader toward a stock pile of material the left front tire ran over a large rock jolting the operator in the cab.

Just as the sun came up truck driver noticed a motionless body lying on the ground at the base of the reclaim pit north high wall. Driver called another employee and they inspected the deceased body, determined the person was not an employee. 2nd employee drove out of the pit to a location where they could get a signal and called me. I immediately called 911 and the MSHA hotline.

Fall to the walkway or working surface

While using a com-a-long to lift a screen deck the handle broke on the com-a-long causing the employee to fall backwards and in doing so EE's left are flailed to catch self and EE hit EE's arm/wrist on the cat walk railing displacing the distal radius.

The miner was using a hand grinder with right hand to smooth out an area on a piece of metal when the grinder at some point grabbed the metal and jumped to the left nicking the middle finger of the left hand of the miner which was bracing the metal.

Employee was pounding on a ripper tooth with a hammer trying to dislodge it from the ripper on a D11 dozer when a small piece of metal broke off the tooth and hit the employee in the lower leg. Mechanic was driven to the emergency room where it was determined to leave the metal in the leg and apply 2 stitches. The mechanic returned to full duty the next day.

Miner was checking a head pulley bearing on a stacker belt against his outlined duties and without prior instruction when he got his had caught between the pulley and the wiper causing a severe crushing injury to his right hand.
