Injured worker was pulling on a large wrench trying to remove a nut. The wrench slipped off of the nut throwing the worker into a metal support causing the injury.

After medical investigation it was determined that the employee slipped/fell from a ladder causing primarily acute back pain and secondary rt. shoulder strain.

On 12/28/2009 we were notified that the injured worker was breaking ice with a chisel and hammer. The injury was caused by a repetitive motion.
Don't know how the accident happened, was welding prior during the day. Never felt anything wrong. Then his eye started to have discomfort (at 4:00 pm).

EE WAS CUTTING THE ANGLE IRON AND IT DROPPED TO THE GROUND. AFTER IT FELL A PIECE NICKED HIS SHIN. HE NEVER REPORTED IT, BECAUSE IT WASN'T A BIG CUT. 2 WEEKS LATER A LIVER OF METAL GOT INFECTED IN HIS LEG.
