EMPLOYEE WAS STANDING BESIDE AND OFF TO ONE SIDE OF JAW CRUSHER WHICH WAS PLUGGED WITH A LARGE CHUNK OF CONCRETE. AN EXCAVATOR WAS PICKING THE PIECE OF CONCRETE OUT OF THE CRUSHER. IT COULD NO T PICK THE PIECE UP HIGH ENOUGH. IT WAS THEN SENT THROUGH AGAIN. A PIECE BROKE OFF & WAS FLUNG BY THE BULL WHEEL. PIECE HIT EMPLOYEE IN HEAD. HESPENT NIGHT IN HOSPITAL & WILL BE OFF UNTIL 8/9

EMPLOYEE WAS ON LADDER WITH PRYBAR THE BOTTOM OF LADDER SLIPPED IN LOOSE DIRT HARRY FELL BACKWARDS OFF LADDER LANDED ON HIS BACK THE PRY BAR THEN LANDED ON HIS FACE CAUSING INJURY.
