EE STARTED EXPERIENCING RIGHT SHOULDER PAIN WHILE LIFTING 40LB BAGS OVER HER HEAD ONTO PALLETS IN DEC, 1997. SHE NOTIFIED THE PLANT MANAGER ABOUT IN ON MARCH 11, 1998 WHEN THE PAIN PROGRESSED TO HER RIGHT ARM AND HAND . SHE WENT TO THE ER THE NEXT DAY AND WAS ADVISED TO NOT LIFT OVER 20LBS FOR 5 DAYS AND TAKE ANTI-INFLAMATORIES.

THE INJURED EE WAS STRETCH WRAPPING A PALLET OF CAT LETTER. A CORNER OF THE PALLET WAS HANGING OVER THE EDGE OF THE TURNTABLE. AS THE PALLET CORNER CAME AROUND, THE CORNER OF THE PALLET PASSED OVER EE'S FOOT & PINCHED HIS FOOT BETWEEN THE PALLET & THE FLOOR & PULLED THE EE ABOUT FIVE FEET BEFORE HIS FOOT WAS RELEASED FROM THE PALLET.
