Razor knife was stuck in the wire nut. When employee was trying to dislodge the knife, it sliced their forearm. Employee had to have six stitches.

While entering EE's truck driver felt EE's left knee pop followed by instant pain. EE was unable to stand or walk on left leg following the incident. EE was diagnosed with a sprain of unspecified collateral ligament of left knee and placed on restricted duty.

THE EMPLOYEE WAS CLEAINING DIRT OFF AROUND THE TAIL PULLEY OF A CONVERYOR BELT. THE PLANT WAS POWERED OFF, LOCKED OUT AND TAGGED OUT. THE BELT UNEXPECTEDLY ROLLED BACKWARD, CAUGHT THE EMPLOYEES GLOVE WHICH PULLED THEIR HAND AND ARM UNDER AND AROUND THE TAIL PULLEY. ANOTHER EMPLOYEE WAS ABLE TO GET A WRENCH AND ROLL THE TAIL PULLEY FORWARD AND RELEASE THE HAND AND ARM.
