EMPLOYEE REACHED OVER TO OPEN DOOR OF WATER TRUCK HIS LEFT FOOT SLIPPED, HIS WHOLE BODY FLEW INTO THE DOOR CRUSHING HIS RIGHT HAND. RIGHT RING FINGER HAD FRACTURED BONE.
