On 9/8/16 employee demonstrated a STS, left ear. Employee stated that they had a double ear infection. As per MSHA and IHT employee retested on 10/5/16. IHT sent a letter stating they couldn't verify the results due to an obstruction of wax in ear. We had employee tested again on 10/13/16 where they again demonstrated the STS. I received the results of that test today 10/25/16.

Fall from ladders

Was working on ladder repairing a spray hose and fell off of ladder, dislocating left wrist.

Employee was guiding a piece of equipment into place during a repair and pinched the skin between the thumb and forefinger. He received a relatively small laceration that required 4 stitches to close.

Struck by falling object

Miner was changing a tooth on a CAT 990 loader bucket when it dropped on his foot, striking it along the edge of the steel toe. The miner suffered a broken toe on his left foot.

While leaving EE noticed his knee was stiff and a bright red color. EE called me and we decided to see how it was the next day. EE could not recall a specific incident where he might have injured it. The next day it was still bothering him and he went to the doctor. The doctor diagnosed EE with prepatella bursitis with possible cellulitis (infection inside the knee).

Employee was changing the backup alarm on the PC-400 Komatsu excavator and was under the machine when he stood up and hit his head. He received a small cut on the top of his scalp that required four stitches.

EE was handling a large equipment jack when it slipped and cut his right middle finger. He recieved a couple of stitches and returned to work.

Employee was welding when he needed to chip away some slag from his weld. A peice of slag flew into his eye. He claims he was wearing safety glasses.

THE MINER WAS EXITING THE CAB OF THE CRANE WHEN HE SLIPPED AND HIT THE OUTRIGGER AND THEN THE GROUND. HE NEVER LOST CONSCIOUSNESS.
