Employee was lifting small pump to set it. Back onto the work bench. He felt immediate sharp upper back and neck pain.

Employee and co-worker were installing a new truck access safety platform. The hinge bracket had not been welded at the factory. (Manufacturer's defect). Hinge bracket broke free. Walkway tipped and fell to ground with both individuals falling with it.
