Cloudy 38 degrees light rain, employee tripped on a forklift fork and fell breaking a bone in left hand.

Plant surface was wet from evening dew. Causing employee to slip. Employee didn't miss work until 10/16/18, surgery.

Employee was carrying 40 pound bags of Floc, when he was putting bag into metal container he felt a pop in his left shoulder. Employee did not feel any discomfort until waking up the next day. Light duty work load until 7/22/2014 will be out of work for about four months due to operation.
