THE GENERATOR PART OF THE ORE HOIST MG SET BROKETHE MAIN SHAFT. THIS BREAK MADE THE MG SET INOPERATIVE & AS A RESULT THE ORE HOIST COULD NOT BE USED.

THE VEHICLE WAS CHOCKE D WITH WOULD NOT ALLOW THE VEHILE TO LUNGE IN REVERSE BUT HE LOST HIS BALANCE & HAD HIS LEFT FOOT UNDET THE SEAT-HIS BODY WENT FORWARD & HE HYPEREXTENDED HIS KNEEE

THE EMPLOYEE WAS TRAMMING A ROOF BOLTER WHEN HE HIT A BUMP IN THE FLOOR. HIS RIGHT FOOT SLIPPED OFF THE FOOTREST AND WAS CAUGHT BETWEEN THE BOLTER FOOT REST AND THE FLOOR BREAKING HIS LEG ABOV E THE KNEE. I MEAN ABOVE THE ANKLE.
THE MINER CREW MOVED THE MINER AND PULLED THE SLACK CABLE OUT WITH A JEEP. AS THE EE WAS THROWING THE CABLE LOOP AGAINST THE RIB WITH A ROPE, HE FELT A PULL IN HIS LOWER ABDOMEN. THE EE CONTIN UED TO WORK UNTIL HE WENT TO A DR ON 9-9-98. AT WHICH TIME HE WAS DIAGNOSED WITH A HERNIA.
THE EE WAS USING A SLEDGE HAMMER TO BEAT ON A PLUGED CHUTE AND SMASHED HER RIGHT INDEX FINGER BETWEEN THE CHUTE AND HAMMER. 4 STITCHES WERE REQUIRED ON HER FINGER. SHE RETD TO HER JOB AT FULL CAPACITY THE NEXT SHIFT
THE EE WAS TAKING DOWN OLD CURTAINS BY HAND AND AS HE PULLED ON A CURTAIN HE APPARENTLY WAS STANDING IN AN ANGLED POSITION AND PULLED A MUSCLE IN HIS BACK Y

THE SUPERINTENDENT WAS DOWN IN THE ORE POCKET DUE TO A SPILL. AS HE WAS COMING OUT OF THE POCKET ON A BELT MANTRIP, HE BEGAN TO FEEL DIZZY AND BECAME UNCONCIOUS, FALLING APPROX 25' TO 30'.

WHILE LOADING BAGS OF NITRATE TO PUT IN NITRATE TANK, TWO BAGS WERE STUCK TOGETHER AND AS HE LIFTED HE FELT A GIVE IN GROIN AREA-EMPLOYEE CONTINUED TO WORK HIS REGULAR JOB UNTIL 5-9-96 & IT WA S DETERMINED HE HAD A HERNIA. 5-9-96 WAS THE EMPLOYEE'S FIRST DAY LOST.
EMPLOYEE WAS HELPING MOVE BOLTER CABLE. HE TRIPPED AND FELL CATCHING HIMSELF WITH HIS ARMS TO AVOID INJURY BUT ENDED UP HAVING A HAIRLINE FRACTURE TO THE BONE IN HIS RIGHT HAND BELOW THE ELBOW . THE EMPLOYEE WAS ON HIS SCHEDULED DAY OFF TIL THURSDAY 5-2-96 - FIRST DAY LOST.
THE EMPLOYEE WAS WORKING ON A LOADER CHAIN. AS HE STRUKC THE PIN TO LOOSEN THE CHAIN A PIECE OF METAL BROKE OFF THE HAMMER STRIKING THE EMPLOYEE IN THE LEFT FOREARM. NO LOST TIME NOR RESTRICTE D WORK OCCURRED.

EMP WAS HAULING PIECE OF POLY PIPE ON TOP OF VEHICLE. VEHICLE WAS DRIVEN TOO CLOSE TO A PARKED MANTRIP AND POLY PIPE CAUGHT MAN TRIP CAUSING PIPE TO WHIP AROUND, STRIKING EMP ON RT SIDE OF SAF ETY GLASSES CAUSING SAFETY GLASSES TO LACERATE ABOVE THE RT EYE, REQUIRING A COUPLE OF STITCHES.
EMPLOYEE ALLEGES THAT WHILE HE WAS LOADING A PIECE OF PLYWOOD ONTO A TRUCK, HE PULLED A MUSCLE IN THE RIGHT ARM.
UPON START UP OF THE LANG PLANT WEST DRYER, THE DRYER WAS LIT SEVERAL TIMES AND PURGED TO ALLOW GAS TO ESCAPE. IT APPEARS THE DAMPER ON THE EXHAUST SYSTEM WAS CLOSED TOO TIGHT NOT ALLOWING A D RAFT OF AIR AND PURGING OF SYSTEM, CAUSING AN IGNITION. THIS INCIDENT WAS IMMEDIATELY REPORTED TO MSHA FIELD OFFICE.
EMPLOYEE WAS OPERATING A ROOF BOLTER SCALING & BARRING DOWN HECARRIED DOWN HOSE SLAB FROM BACK & AN ADJOINING 2IN PIECE FELL INEXPEDITELY CAUSING HIS HARD HAT TO PRESS DOWN ON SAFETY GLASSES C AUSING A CUT ON THE BRIDGE OF NOSE REQUIRING 2 STITCHES NO TIME WAS LOST AS A RESULT OF THIS INCIDENT
THE EMPLOYEE WAS RUNNING AN UNDERCUTTER. AS HE STEPPED OUT OF THE OPERATOR'S CAGE HE STEPPED ON A SMALL BOULDER, LOST HIS BALANCE AND HIT HIS HEAD AGAINST THE RIB, CAUSING A STIFF NECK.
EE WAS RUNNING A LOADING MACHINE AND WAS OBSERVED LIMPING ON 4-2-95. EE CONTINUED WORKING ON LOADER THRU 4-27-95 AND CLAIMED MINING TIGHT MUCK CAUSING LOADING MACHINE TO BRUNCE CAUSING SORE BA CK. EE FIRST DAY LOST WAS 4-28-95 FOR A SORE BACK ATTIBUTED TO BOUNCING LOADER.
THE EMPLOYEE WAS SERVICING EQUIPMENT. AFTER SERVICING HE STARTED TO REEL UP HIS GREASE HOSE. HE HAD A LONG LENGTH OF HOSE OUT AND STATED HE WAS AFRAID THE HOSE WOULD WHIP SO HE GRABBED THE REE L . AS IT WAS TURNING AND THE MOMENTUM OF THE REEL CAUSED HIS LEFT HAND TO STRIKE THE REEL END. THE EMPLOYEE HAS CONTINUED TO WORK AND HAS NOT LOST ANY TIME.
THE EMPLOYEE WAS WALKING DOWN STAIRS. STAIRS WERE WET & FROSTED DUE TO RAIN & TEMPERATURE & HE SLIPPED & FELL TO HIS KNEES BRUISING HIS RIGHT KNEE.
THIS MECHANIC WAS HELPING LIFT SECTIONS OF PIPE ONTO A TRAILER IN ORDER TO TRANSPORT PIPE TO NEWSHOP. AFTER LIFTING SECTIONS SEVERAL DAYS LATER HIS BACK BECAME SORE. THE EMPLOYEE CONTINUED TO WORK AT HIS NORMAL JOB UNTIL 2-6-95 FIRST DAY LOST.
THIS MECHANIC WAS HELPING JACK UP A LOADER. THE JACK WAS AT IT'S HEIGHT AND HE WANTED ONE MORE CLICK UP ON THE JACK. HE LET LOOSE OF THE JACK HANDLE WHICH WAS SNAPPED ITNO POSITION. THE CA TCH SLIPPED CAUSING JACK HANDLE TO FLY UP STRIKING EMPLOYEE IN THE CHEEK.

THE EE WAS BARRING DOWN A SLAB. AS HE EXERTED PRESSURE UPWARDS ON THE BAR, HE SUSTAINED A HERNIA. THE EE CONTINED TO WORK UNTIL 11/4/93, WHICH WAS THE FIRST DAY OF LOST TIME.
THE EMPLOYEE WAS HELPING INSTALL A FUEL TANK IN AN MG SET. AS THE TANK SLIPPED INTO POSITION, THE EMPLOYEE'S SECOND INDEX FINGER OF HIS RIGHT HAND GOT PINCHED BETWEEN THE FUEL TANK AND FRAME OF THE UNIT AND THE FINGER WAS FRACTURED.
EMPLOYEE WAS CUTTING STEEL CABLE FRO A NEW BELT LINE SUPPORT WITH A CABLE CUTTER. AS HE STRUCK THE CUTTER WITH THE HAMMER, A PIECE OF WIRE BROKE OFF, PENETRATING THE SKIN BY THE LEFT SIDE OF T HE NOSE.
EMPLOYEE PARKER HIS SHUTTLE CAR AND WAS UNPLUGGING HIS CABLE NIP. AS HE PULLED ON THE CABLE, THE BOX AND RIB PEELED OFF, STRIKING THE EMPLOYEE IN THE RIGHT SHOULDER.
EE WAS LOADING A FACE IN 5-EAST 1ST LEFT. HE LEFT HIS KNIFE OPENED AND CLIPPED TO HIS BELT. HE WIPED HIS HAND DOWN HIS PANTS, MAKING CONTACT WITH THE KNIFE, CUTTING HIS RIGHT THUMB AND REQUIRI NG STITCHES.
EMPLOYEE WAS PULLING ON A PIN WITH A PAIR OF CHANNEL LOCKS. THE CHANNEL LOCKS SLIPPED AND HIT HIM ON THE MOUTH, BRAKING A FRONT TOOTH. THE PIN WAS GREASY, WHICHCONTRIBUTED TO CHANNEL LOCKS S LIPPING OFF PIN.

THE #6 HOIST WENT OUT OF SERVICE AT APPROX. 3:00PM ON DEC 18, 1992. THE #6 HOIST WOULD GO DOWN BUT NOT COME UP. THE PROBLEM WAS FRUND IN THE MAIN CONTACTOR RETIFIER. A DIODE WAS FOUND TO BE B. O., REPLACED AND THE HOIST RESUMED NORMAL OPERATION.
THE #5 HOIST WENT OUT OF SERVICE AT 4:30 PM ON 09-24-92 & WAS PUT BACK IN SERVICE AT 11:30PM ON 09-24-92 #1 SKIP WOULD NOT LIFT OUT OF THE BOTTOM OF THE SHAFT BECAUSE OF ELECTRICAL PROBLEMS PR OBLEM WAS IN THE PPM MODULE AFTER CHANGING OUT SEVERAL COMPONENTS & MAKING SOME ADJUSTMENTS THE HOIST RESUMED NORMAL OPERATION
EMPLOYEE WAS TAKING DOWN VENT TUBING IN HIGH BACK, WORKING OFF TOP OF VENTILATION TRUCK. HIS FEET BECAME ENTANGLED WITH THE TUBING, CAUSING HIM TO TRIP AND LAND ON A CUTTING MACHINE PARKED NEX Y TO VENT TRUCK.
EMPL LAYING UNDER POWDER TRUCK PUTTING A GROUND STRAP ON IT,GOT UP,RIGHT KNEE WOULDNT STRAIGHTEN AND IT POPPED.
HOIST WENT OUT OF SERVICE DOOR REMAINED OPEN
WORKING ON STAMLER: EMPLOYEE SWUNG HIS LEG OVER THE EDGE OF STAMLER (RATIO FEEDER) TO GET OUT. HE BRUISED HIS LEFT LEG (CALF) SLIGHTLY AND ABOUT 10 DAYS DAYS LATER HIS LEG SWELLED UP AND THE D OCTOR"S DIAGNOSIS WAS PHLEBITIS (BLOOD CLOT).

BEFORE SHIFT HOISTMAN WHILE CHECKING SAFETY DEVICES NOTICED HOIST WOULD KICK POWER ON SPPED CONTROL. TVA SPEED REGULATOR HAD DEFECTIVE DIODE OR THYRISTOR & COMPLETE UNIT WAS CHANGED OUT. NO EM PLS UNDERGROUND & NO ONE WAS ALLOWED UNDERGROUND WHILE HOIST BO
EMPLOYEE STATED HE WAS ATTEMPTED TO NIP THE UNDERCUTTER INTO THE POWER CENTER. HE STATED HE TWISTED HIS BACK AND AS A RESULT HE RUPTURED TWO DISC IN HIS BACK.
EE WAS DRIVING THE UTILITY TRUCK PULLING THE MANTRIP UP THE SLOPE. A PARTICLE OF SALT GOT IN HIS EYE AND CAUSED A REDNESS. THE DR PRESCRIBED A PRESCRIPTION OF SALVE.

A WELDER AND A MECHANIC WAS USING A TORCH AND PULLER TO REMOVE A BEARING FROM A SHAFT. WHILE THE WELDER WAS HEATING THE BEARING ONE OF THE PULLER BOLTS BROKE CAUSING THE PULLER TO SWING AROUND STRIKING THE EMPLOYEES RIGHT HAND. THE THUMB ON THE RIGHT HAND WAS FRACTURED.
EMPLOYEE WAS USING A CHEATER PIPE TO LOOSEN A BOLT ON A JACK SHAFT. HE PUSHED TO HARD STRAINING HIS RIGHT SHOULDER. A PRESCRIPTION WAS GIVEN.
EMPLOYEE WAS LIFTING A 50 POUND BAG OF NITRATE TO DEPOSIT IN A HOPPER ON THE POWDER TRUCK EMPLOYEE TURNED WITH BAG IN HAND & FELT A MUSCLE PULL IN HIS BACK EMPLOYEE WAS GIVEN A MUSCLE RELAXER
EMPLOYEE WAS UNLOADING BELT RETURN ROLLERS FROM SKID TO BE INSTALLED ON NEW BELT.EMPLOYEE MASHED LEFT FINGERS ON BELT FRAME.CAUGHT HAND BETWEEN ROLLER AND BELT FRAME.DOCTOR PRESCRIBED A PRESCR ITPTION.
EMPLOYEE WAS DISCONNECTING MANTRIPTRAILER, WHEN HE LET TONGUE OF TRAILER FALL,STRICKING HIM ON THE FOOT, ABOVE HARD TOES. THE FALL BRUISED HIS RIGHT MATERTARSAL.
EMPLOYEE WAS FILLING UP HOOPER OF POWDER TRUCK WITH AMMONIUM NITRATE. EMPLOYEE SLIPPED, CAUSING HIM TO STRIKE THE POWDER TRUCK, RESULTING IN BRUISED RIBS. EMPLOYEE WAS RELEASED TO GO BACK TO W ORK. HE WORKED ONE DAY, CALLED THE COMPANY DR. ON 3/9/89, AND STATED HIS RIBS WERE SO SORE, HE COULDN'T WORK. (FIRST DAY OFF WORK WAS 3/9/89).
EMPL WAS REMOVING STEP JACK FROM UNDER DRILL AFTER CHANGING TIRE. HEAVY LAOD CAUSED JACK TO KICK BACK STRIKING EMPL IN RIGHT CHEEK.
EMPLOYEE ALLEGES THAT WHILE USING FOAM MACHINE TO SPRAY VENTILATION WALLS, FACE MASK CAUGHT ON A "J" HOOK, CAUSING HIM TO TWIST HIS NECK. THIS HAPPENED JANUARY 7, 1989. DR. NOW STATES HE HAS C RUSHED DISC IN HIS NECK. FIRST DAY MISSED FROM IS 5/11/89.

EMPLOYEE WAS STANDING ON BATTERY BOX OF HYDRAULIC CONTROLLED JEEP (GOPHER), PULLING THE SLACK OUT OF A LIGHT LINE HE HAD JUST INSTALLED, WHEN HE LOST HIS BALANCE AND FELL, STRIKING THE GEAR SHIFT. THE GEAR SHIFT DID NOT HAVE A KNOB AND IT PUNCTURED HIS LEG. STITCHES WERE REQUIRED.
SUPERVISOR PUT FINGERS IN HOLES OF SPOOL OF SHOT WIRE TO UNWIND WIRE. THE SHARP EDGES OF SPOOLS CENTER CUT HIS LEFT RING FINGER. THE EMPLOYEE RECEIVED TWO STITCHES.
THERE WAS A DIESEL VEHICLE PARKED ALONGSIDE THE RIB IN THE MAIN BELT HAULAGE. THE MECHANIC PULLED OUT TO GO AROUND THE VEHICLE HITTING THE ROPE BELT AND GOING UNDER IT. THE MECHANIC'S SHOULDER WAS THEREAFTER SORE AND STIFF.
EMPLOYEE PUT ON WELDING GOGGLES, LAID ON HIS BACK TO WELD UP, AND ROEIGN OBJECT ENTERED EYE. DR. RMOVED SMALL FOREIGN MATTER FROM EYE LID WITH COTTON SWAB. FOREIGN MATTER WAS IN GOGGLES WHEN HE PUT THEM ON. PRESCRIPTION EYE DROPS WERE DISPENSED BY DR..
POWDERMAN SHOT FACES AND A SMALL SLAB (ABOUT 1/2 POUND) FELL FROM BACK STRIKING EMPLOYEE ON FINGER WHILE HIS HAND WAS STILL ON METAL BLASTING GUN. THE LAST JOINT OF HIS INDEX FINGER ON LEFT HAND WAS SPLIT (FLESH ONLY). THE EMPLOYEE REQUIRED A PRESCRIPTION AND A SHOT.
THE #5 ORE HOIST HAD AN ELECTRICAL FAILURE AT 12:15 PM ON WEDNESDAY MARCH 23 1988.THE POWER RESISTOR THAT CONTROLS THE MOTOR FIELDS COILS OPENED.THE HOIST WAS BACK IN OPERATION AT 1:45 PM ON M ARCH 23,1988.
EMPLOYEE WAS BARRING DOWN WHILE ELECTRICIAN WAS WORKING ON CUTTER CABLE. ELECTRICIAN NIPPED CUTTER CABLE UP AND TURNED POWER ON. THE PUMP SWITCH WAS STUCK IN THE ON POSITION, WHICH CAUSED CABL E REEL TO PICK UP AND STRIKE EMPLOYEE ON THE SIDE OF THE KNEE, RESULTING IN POSSIBLE LIGAMENT DAMAGE.
AFTER PULLING S5IPS OF ORE, THE HOIST CONTROL CIRCUIT WENT OFF. THE WIRE OF THE LILLY OVERSPEED SWITCH BROKE WHERE IT ENTERED THE LUG. THIS WAS REPLACED WITH A MORE FLEXIBLE PIECE OF WIRE AN D THE HOIST WAS3PUT BACK IN SERVICE.

HE DID NOT BELIEVE THAT SKIPS WERE AT CYCLE TERMINUS AND ATTEMPTED TO BACK 2 OUT OF POCKET & UP TO SYLVITE LEVEL

EMPLOYEE WAS WO9KING ON CONVEYOR FLITE OF SHUTTLE CAR. MASHED RIGHT THUMB BETWEEN FLITE AND FRAME OF CAR. BROKE TIP OF BONE.

EMP WAS CHISELING THE RIB WITH SCREW DRIVER AND HAMMER TO MOVE CABLE FROM BETWEEN THE RIB AND SHUTTLECAR THAT WAS PARKED FOR REPAIRS EMP MISSED SCREWDRIVER AND HIT HIS LITTLE FINGER ON LEFT HA ND

INSTALLING CUTTER BAR AND CHAIN ON UNDERCUTTER.-CONNECTING CUTTER CHAIN TOGETHER,USING ALIGNMENT PIN,PIN AND HAMMER,PIECE OF STEEL BROKE OFF FACE EDGE OF HAMMER WHEN STRIKING PIN AND PENETRAT EMPL THUMB.
EMPLOYEE WAS REPLACING LINKS IN A SHUTTLE CAR CONVEYOR CHAIN HE HAMMERED ON VERY HARD AND BRITTLE STEEL WITH HAMMER AND PUNCH PIECE OF STEEL FLEW UP AND WAS IMBEDDED JUST BELOW RIGHT EYE
EMPLOYEE WAS DRIVING SCOOP TO CLEAN OFF CRUSHER. HE DROVE IT UNDER THE CRUSHER POWER CABLE & SCOOP CAB HUNG ON CABLE. HE THEN REACHED UP TO FREE CABLE, FOOT PUSHED ON ACCELERATOR WHICH TIGHTEN ED CAB UP ON CABLE, AND FINGER WAS CAUGHT BETWEEN CABLE AND CAB, MASHING IT.
