On 08/25/21, about 1:00 PM I was trying to climb down from the Loader and must have missed a step. I slipped down off of the loader and hit my head. My coworkers working nearby ran over and revived me and then called 911. An ambulance came & I went to GIMC where they ran tests & I stayed until almost 10:30 that night.

Was trying to reset a circuit box that had a broken handle - used a metal bar.

Conveyor stopped running. Employee replaced fuses and turned it back on. Conveyor didn't start. He pulled disconnect box off. Electricity arched on switch blades causing the box to blow up.
Rock stuck in conveyor. Employee was prying on it with a bar. Bar slipped catching his finger between bar and crusher frame.

EMPLOYEE WAS PUTTING THE BACK STOP ON A GEAR BOX. HE TUGGED ON THE CONVEYER BELT AND IT JUMPED FORWARD ABOUT 2" CATCHING HIS FINGER BETWEEN THE BELT AND SHIVE. CONVEYER WAS NOT HOOKED TO POWER . EE'S FINGER WAS X-RAYED AND WAS NOT BROKEN, JUST SORE. HE IS O.K. AS OF TODAY. NO LOST TIME.

CLIMBED DOWN TOWER LADDER VERY HIGH WINDS BLEW ASMALL WINDOW PANE OUT OF WINDOW. EE HEARD SOMETHING & LOOKED UP AND SAW GLASS COMING. HE THEN DUCKED & GLASS HIT HIM IN THE BACK OF THE HEAD BEL OW THE HARD HAT CUT HIS HEAD. HE GOT 5 STAPLES IN HIS HEAD
