Struck against stationary object

While walking to enter forepersons pickup truck, miner was watching footing and not looking ahead. Miner struck forehead on corner of pickup truck forcing hard hat up and off of head and causing laceration to forehead. 8 stitches required; employee released to full duty same day.

Rubbed or abraded

Mechanic removed cutting edge from loader bucket. Following removal and placement of old cutting edge inside bucket, they pushed the cutting edge out of the way. In doing so, the edge lacerated glove through to left index fingertip. 4 stitches required.

The employee retrieved overalls and was walking away from personal vehicle when they stepped on a patch of ice. Employee lost footing, slipped and fell landing on left shoulder.

While using a push scraper to clean up he unexpectedly hit the edge of a metal plate. The scraper kicked back and the handle hit his left cheek causing a left bottom tooth to break and need removal.

While operating a haul truck debris came out of heater vent and imbedded in eye.

Employee exited catwalk and climbed down ladder. He hit his right knee on a hook that used to hold a hanging ladder. The hanging ladder had been removed from service but the hook wasn't. The hook fell between the rungs of the ladder the employee was using. The hook had a sharp edge that lacerated employees knee.

Employee states he had pain in shoulder for past 7-8 months and didn't report it. He couldn't stand it anymore and went to the doctor on 9/17/12. Doctor diagnosed bursitis from repetitive overhead motion at work.

Employee was using punch on belting, he let go of punch after he had it seated and then hit it with a hammer. The punch came back up and hit him in the left cheek causing a laceration requiring 5 stitches

Employee had lifted hard hat to wipe sweat from brow. Stone came off of catwalk went behind his safety glasses and hit lower lid of left eye causing a contusion that required a prescription

Employee was using hammer to hit a chisel, head of hammer came free. Head then hit side of concave ring, deflected off ring hitting employees leg. Employee states he received a small scrape. He didn't initially report incident. Date of initial injury is unknown date filed is approx. The scrape became infected and employee then notified his supervisor on 2/8/2012

While changing out a screen, employee hit his hand on a dust cover that came loose. He attempted to grab it and instead, he caught his finger between it and the screen, causing a laceration.

Claims he unbuckled seatbelt ,while haul truck was being loaded with shot rock, to get a water bottle. The force of the rock being loaded caused him to be bounced out of the seat and he hit his head on the top of the cab. Strained neck muscles. Also claims he hit his head earlier on a piece of conduit he forgot to duck.
