Employee was inspecting the K1 front clinker tunnel to ensure that the other shifts have been doing there assigned tasks. During inspection they found that both the north and south side of the tunnels had spilled material so they went ahead and started removing the build up by shoveling when they encountered a sharp stabbing pain in lower abdomen right around the belly button.

Employee was operating the primary plant when the fines belt unexpectedly stopped. As EE made way to shut off the screen deck to avoid the overflow of material, EE lost footing and fell forward on to their hands. This is when EE felt a pain and strain in the lower back.

Employee was completing maintenance & repair on the Kiln 2 Clinker Conveyor when EE's left hand got sucked into the troughing roller and became entangled between the belt and the roller.
Employee reports that was walking, slipped on gravel and slid into guardrail on roadway. Managed as first aid initially, latest follow up appointment 12/2/2022, Physical Therapy was ordered and also prescription medication along with work restrictions. Note: Because employee is a manager the restrictions didn't affect the ability to do the job at full capacity.
A Finish plant employee had dust get into eyes causing irritation. This was reported on Monday morning 8/9/22. One source was called and EE was sent to the Eye Doctor for first aid and evaluation. Had follow up and was given steroids to help with the inflammation.
Zampell employee was attempting to clean sensors on shotcrete machine, in order to do this, the lid must be fully extended and secured via the locking mechanism. The employee had not verified the locking mechanism was set and began to clean sensors. The lid came back down and hit the employee's finger causing a fracture and cut to the employee's index finger.
Maintenance Manager bent over and picked up a 5 gallon pail filled with liquid when EE felt pain in the lower back. Physical therapy was prescribed as a result.

Employee was clearing material from APC Tower when EE felt a sharp pain in lower back and down right leg. Employee was seen at the clinic, given work restrictions for light duty and prescribed muscle relaxer.
Employee reports was climbing off a front end loader and fell 3-4 ft onto left knee.
Employee was driving a F150 truck at approximately 5mph and crash into a beam causing their face to hit the steering wheel. Complains of face and neck pain. X-ray of the c-spine performed and diagnosed with Cervical strain. Return to work light duty.
Fuel Farm Employee was moving 55 gallon drums of hazardous waste onto a pallet by hand and a finger got caught between 2 drums.
Employee reports EE was checking a hopper and stepped on a piece of aggregate material while taking a step back. Employee reports hearing a pop and twisted EE's left knee. Employee was put on restrictions and referred to an orthopedist after being diagnosed with a left knee sprain.
Employee reports they were dragging hoses upstairs and twisted left knee. Employee reports they felt a pop. The employee was seen at the clinic and put on restricted light duty for the sprained knee.
Employee reports that while washing a truck employee picked up a hose and felt a pain in back. The employee reported the incident 2 days later to manager 03/04/2021 and requested to be seen at the clinic where employee was given work restrictions.
Employee reports they slipped on concrete and fell. 3/12/201 employee sent for MRI of Left knee- Grade 2 MCL sprain. Restriction imposed 3/12/2021 of Desk duty only.
Miner was operating a mobile industrial tool. The tool recoiled, striking miner in the upper lip and front tooth.

While tightening a nut & bolt with a socket wrench on a flange, the wrench slipped causing the employee to strike the left hand on a piece of metal conduit nearby. Employee reported the incident the next morning due to some swelling on the pinky and ring finger. The employee was seen at the clinic and X-Ray was taken confirming a fracture on the pinky of the left hand.
The employee was pulling welding leads up to the 4th level of the APC tower, and the welding lead separated at the joint connector. When the lead separated, the connector flew up and hit the employee in the mouth breaking two upper front teeth, and cutting their lip. The employee was seen at a dentist for evaluation on 10/27/2020 where one of the teeth will require an implant.
The employee was performing a bucket calibration test on the kiln 1 and 2 lime silos. A trash bin had been filled with lime and was being moved to a scale to be weighed, when the trash bin was set down - lime puffed up from the bin and got behind EE's safety glasses and into EE's left eye. Employee was seen at ER and prescribed antibiotic eye drop.
Employee was removing old flood lights from the lower left corner of the kiln 1 feed pier. While on an extension ladder, the employee shifted weight causing the ladder to shift, and employee lost balance and fell striking face on the ground. Employee was treated at the E.R. for a laceration and broken nose.

The employee was pulling plate steel from cooler wall when the plate broke free and struck an employee in the forehead. 4 Stitches were received.
EE was walking down roadway from job site trailer to the working area when EE slipped on ice and fell.
While cleaning a material buildup in a chute with a scraper bar, the employee felt pain in the right shoulder. The scraper hit material in the chute that caused the employee's shoulder to ache. This incident was treated and managed as a first aid until physical therapy was prescribed upon a follow up appointment. 11/20/2019 PT prescribed as medical treatment.

Employee was attempting to open a door. When the door wouldn't open, ee stepped up onto a pipe. Upon stepping back down, ee rolled ankle.
The mechanic was using a 4# sledge hammer to drive a new pin for the spade bucket tooth of the 988B Front End Loader when hammer contacted the employees left pinky finger. Bleeding and swelling to the pinky was noticed immediately and the employee was taken to the clinic. The wound was stitched up, a rigid splint was issued and the employee was referred to an Ortho.
The employee was removing a knife gate plate for Kiln #1 shale feeder when the left hand slipped and contacted the steel feeder. The left knuckle and pinky showed swelling and the employee was sent to the clinic to be evaluated. The X-Ray showed a fracture to the left pinky finger, a splint was applied and the employee was given work restrictions on left hand.
The employee was pulling upward to remove a bag and cage during routine maintenance when the bag/became hung up and didn't budge. The employee felt a strain to the left lower back.
The employee was operating a Vac truck when the employee slipped exiting the truck, outstretching the right arm to try and catch self as the employee fell to the ground. The employee felt pain in the right shoulder and was sent out for evaluation at the clinic. Restricted duty and referral to physical therapy.

The employee hit the left shin and suffered a contusion, treated with first aid. During the follow up appointment 12/6/2017, the start of an infection was discovered and the employee was prescribed antibiotics.
The employee was sitting at the desk in the office when ee felt something enter the left eye. The employee was sent to the clinic for evaluation and was diagnosed with a corneal abrasion to the left eye. The employee was prescribed eye drops.
Employee was shoveling material in the Finish Plant for approximately an hour when the left wrist became aggravated with a tingling sensation and numbness. This incident was managed as a first aid initially, 5/22/17, with little improvement. The employee has been approved for physical therapy 06/13/2017 and has work restrictions due to the left wrist.
The Employee was stepping out of the CAT 259D Skid Steer and felt a strain to the right lower back.

Employee was cleaning various pipes and fittings during a kiln outage. Employee lifted the steel recycle basket housing and strained their back. Employee has a pre- existing injury pertaining to the lower back. Was seen by their Physician and placed on light duty 8/30/2016
The employee was walking in the plant when EE stepped into a rut twisting the right knee first aid given ice pack applied and employee declined further medical treatment. July 7 2016 employee requested to be treated and was sent to the WorkFit Medical and put on restricted duty, sitting duty only.
The employee was descending a fixed ladder in the Waste Water Treatment Building when one foot slipped off the ladder rung resulting in a strain to the right shoulder after the incident. The employee was seen at Work Fit Medical on 2/1/2016 and was put on restricted duty for the right shoulder.

Employee was climbing up into the cab of the water truck, grabbing the assist handle with the left hand when he felt a strain to the left upper arm (bicep) area. Employee returned to work with no lost days but restricted to 2 LB lifting on that arm.

Tunnel was full of water from recent rain. Sump pump in the back of the tunnel was flooded. Employee had to pull the pump out when he did he felt his elbow hyper extend.
Driving the skid steer on the uneven ground around the MCC bldg.
Employee was changing out an empty bleach tote. He was unaware the valve was open. He opened the cap, with the valve open this caused the bleach to spill out covering him from head to toe.
While jack hammering in the crusher (primary), the jack hammer jerked causing pain in right hand/wrist
Exiting bobcat and slipped in the mud, twisting his right knee. For the first two days received first aid, Wednesday 9/10/14 received a script for physical therapy. Transferred to light duty until 9/17/14.
On 2/01/14 the employee reported that he had re-injured his Right Shoulder a couple of months ago pulling a dust line. No injury report was made at the time. The same shoulder was previously injured in 2011. He stated the MRI showed a Rotator Cuff tear which would require surgery. On 2/5/2014 he was taken out of work by his doctor pending Right Shoulder surgery.

Employee was walking behind Kiln #1 shale feed conveyor when he stepped in a rut twisting his right ankle. He was taken to the emergency room for evaluation where x-rays revealed a right ankle fracture.
Employee was rolling an empty 55 gallon drum onto a loader bucket to put in the scrap metal dumpster. His glove stuck on the drum pulling him forward and injuring his Right Shoulder
Employee slipped on an icy concrete pad at the Primary Plant and fell injuring his Right Knee. He was taken to the hospital for evaluation and was diagnosed with a Right Knee Strain. He was also placed on restricted duty for 7 days

Employee was exiting the door at Baghouse #1. He stepped down onto a cinder block which was being used as a makeshift step. When he stepped onto the edge of the block it gave way causing the employee to twist his right ankle. He was evaluated by a specialist and diagnosed with a right ankle sprain.
Mechanic was polishing the surface for an old pump with a die grinder when he felt something enter his Left Eye. He could not remove the object with irrigation or a Q-tip. He was wearing safety glasses with side shields but no goggles. An ophthalmologist removed a small piece of rust from his cornea and prescribed antibiotics drops.
Employee states he stepped off the Finish Plant control room platform into a hole created by flooding. In the process he felt pain in the left Achilles area. Originally only fist aid treatment was provided but on 6/26 he was seen by an Orthopedic doctor and the Left ankle was immobilized with a rigid splint.
The employee used a pry bar to act as a breaking system on a fan shaft. At the same time, the employee was attempting to use a pipe wrench to block the shaft from movement. As the employee was applying the pipe wrench, the shaft turned which pinched the employee's right ring finger between the wrench and fan frame. The finger was broke in 2 places. Surgery was on 4/6/12.
The employee was working alone to clear the kiln 1 multiclone rotary valve.The employee exposed the bottom of the rotary by removing the venturi. The employee ran the rotary to clear out pieces of debris. It is unknown why, but the employee placed himself near the running rotary valve. His hand was caught in the valve which severed his thumb, index, and middle fingers.
Employee was pulling a clinker ball out of the grizzly when the ball rolled over the puller bar. His left arm got stuck and he felt a pop in his left shoulder. The next day he was sent for a medical evaluation and he was placed on restricted duty.
The employee was dumping a self dumping hoper. After dumping, one side of the hoper slide out of line. The employee tried to lift the one side to put the hoper back on track. When the employee was lifting, he felt muscles in his stomach pull. Upon further evaluation, it was determined the employee had a hernia which would require surgery for repair. Surgery date: 03/26/12.
Employee was cleaning the K-2 middle chute and was climbing the fixed ladder that goes to the k-2 top belt. His foot slipped on the top rung and he hurt his right shoulder trying to catch himself. He had surgery on 1/25/12 to repair the right shoulder and a torn muscle.

Worker was using a wrench to loosen a bolt, his hand slipped off the wrench handle, causing him to hit himself in the mouth. The hit to the mouth caused the front two bottom teeth to become loose. On 02/21/2007, the worker underwent oral surgery to repair the two loose front teeth.
An employee started down a set of icy stairs and. At the bottom of the stairs the employee caught his foot on a stair and strained his knee. At the time the employee did not want any medical attention. At a later date, the employee filed a workers compensation claim for the injury. Upon further evaluation, the employee was diagnosed with a torn MCL that requires surgery.

Two employees were attempting to install a bearing on a tail pulley shaft. They attempted to "drive" the bearing on with a sleeve and a hammer. One employee was holding the sleeve while the other employee began driving the bearing on with a hammer. The employee swinging the hammer hit the other employee's finger, pinching it between the hammer and the sleeve.
Operator was standing on a platform on a cone crushing plant cleaning a spill plate when he twisted and pulled a muscle in his lower back region.

The employee was descending a fixed ladder when his foot slipped on a ladder rung. He attempted to catch himself by putting his left hand below him on the next rung when he heard a crack in his left wrist region.
Employee was cleaning a chute with a 14'X2" piece of flatstock steel when the steel became stuck in the chute. The employee tugged on the steel, freeing it up. When the steel was finally pulled out, it struck the employees forehead. The laceration required 8 stitches.
Employee tripped on a printer basket in the kiln control room on the date listed above, resulting in some discomfort to his right knee. The employee did not want any medical attention at the time of the injury. At a later date, the employee filed a workers compensation claim for the injury. Upon further evaluation, the employee was diagnosed with a torn ACL that requires surgery.
While making his rounds, a supervisor fell on a small patch of ice behind the wastewater treatment plant. Rock salt applied to patch of ice. All walkways were free of snow and ice buildup at the time. Ice patch was approximately 1 square foot in size.

Employee was removing a 4" vacuum hose from a quick connect fitting that was apparently "stuck". When the employee freed the hose, his right fourth digit got caught beween a conveyor frame and the fitting, causing a fracture.

EE WAS CUTTING A WELD ON A 30" X 8' X 1/2" SECTION OF STEEL PLATE WHEN THE STEEL PLATE MOVED, PINCHING THE EE'S RIGHT FOOT BETWEEN THE WALKWAY AND THE STEEL PLATE. EE WAS WEARING STEEL TOED BO OTS WHICH SAVE ALL OF HIS TOES.
EE HAS BEEN DIAGNOSED WITH POSSIBLE SYNOVITIS OF HIS LEFT KNEE. SYNOVITIS IS AN INFLAMATION OF THE JOINT WHICH WAS AGGRAVATED BY THE OPERATION OF THE LOADER.
EMPLOYEE WAS PREPPING A LAB SAMPLE FOR ANALYSIS USING SULFURIC ACID. WHEN THE SULFURIC ACID WAS INTRODUCED TO THE SAMPLE, A REACTION OCCURRED CAUSING ACID TO BE BLOWN ON TO EMPLOYEE'S FACIAL A REA AS WELL AS BOTH FOREARM AREAS NOT COVERED BY LAB COAT.
EMPLOYEE WAS LIFTING MULTICLONE ROTARY VALVE TO PUT BACK IN PLACE WHEN HE FELT A "PULL" IN HIS GROIN AREA.

EMPLOYEE CLAIMS THAT LIFTING THE DEMOLITION HAMMER, WHICH WEIGHS 30 POUNDS WAS THE PRIMARY CAUSEFOR THE INJURY TO HIS BACK.

EMPLOYEE WAS SENT TO ACCESS HEALTH FOR EVALUATION WHERE IT WAS DETERMINED THAT HIS DISCOMFORT WAS A RESULT OF A GEN GLION CYST, WHICH WAS NOT WORK RELATED.
