Employee struck left hand with a hammer while performing winter plant maintenance.

Fall to the walkway or working surface

Employee slipped and fell while walking around equipment during fueling. Employee's left wrist was injured breaking the fall.

Struck by flying object

After welding on the Omni Short Head crusher, employee was grinding down the weld when a piece of metal slag flew behind EE's safety glasses and landed in the right eye.

Over-exertion in lifting objects

Employee strained EE's lower back lifting 4 inch steel square stock tubing off the ground to place onto a block.

Struck by flying object

Employee was using an impact wrench with a 1 1/4 inch socket when the socket came free from the wrench and struck the employee in the mouth breaking a top, front tooth.

Struck by flying object

The employee was making repairs to an excavator bucket, removing excess welds and slag with a hammer and chisel. When struck, the chisel broke free from it's holder and struck the employee in the mouth cutting EE's upper lip and breaking EE's right front tooth in half.

Employee was removing a large nut from a cone crusher tramp cylinder with a large air gun and heavy socket that fits the nut. When the nut came to the end, the socket fell over onto the employee's finger.

Employee was sitting on bench in break area before shift started. Employee stood up to punch in and turned wrong causing a strain to back. Employee stated that employee sustained an injury to back in the military a few years ago and this aggravated that old injury.

Employee stood up after working on short head crusher. Started walking around the catwalk carrying a wrench in right hand. Right ankle gave out and fell to catwalk causing sprain to right ankle. Walkway was dry and clear of obstruction. There were no slip/trip hazards.

Employee stated was changing screens at the 8x24 location in the plant and felt something in the back as EE was maneuvering the screens into position. * Please note this injury was not reported until July 7, 2017.

Fall from machine

Employee was dismounting from loader to refuel at the end of the day. He thinks his foot slipped as he stepped onto the last step (approximately 18 inches from the ground), lost his balance and fell to the ground.

Struck against a moving object

Employee was raising dump body of dump truck to empty load. Body was approximately 75% raised when hydraulic line burst and dump body slammed back down causing jolt to employee sitting inside the cab.

Over-exertion in lifting objects

Employee was unloading steel plate from forklift when he felt discomfort in lower back during the task.

Employee was holding pin punch by handle while second employee used sledge hammer to strike pin to remove master pin from spare excavator track. Employee felt foreign object break skin and enter left forearm when pin punch was struck.
