Employee's left pinky finger caught between cobble stone and bin chute. Bruise/Sprain, restricted duty.
Employee was exiting stairs, lost balance and slipped to the next step down causing a sharp pain in their lower back.

Employee was changing a front-end loader cutting-edge when a sharp burr on a bolt caught employee's right index finger causing a laceration that required 3 stitches to close.

Employees were installing and seating top ring of cone crusher when employee had EE's right hand struck by a sledge hammer, cutting and breaking EE's middle finger, and crushing EE's top knuckle.

Employee needed to clear a rock that was trapped in the tailgate after dumping a load. EE used EE's hand and pulled on the side pin of the tailgate to open it. The rock cleared sooner than expected and the pin cut EE's finger as the tailgate returned to the closed position. EE sustained a laceration that required stitches.

Employee exited loader after completing snow plowing task during active storm. Employee slipped and fell while walking across yard shortly after exiting the loader, fracturing right leg.

The miner was either entering or exiting the cab of a Terex TA-30 articulated end dump when he either slipped or lost his grip causing him to fall backwards from the truck striking his back on a concrete barrier then landing on the ground.
The miner was kneeling on the ground using a sheet rock knife to cut a piece of new skirt board rubber. The blade slipped along the rubber striking the front of his right thigh causing a cut, which required four sutures to close. The next morning, we disccused what would have prevented the accident and what he could have done to protect himself against being cut.
