Struck against stationary object

Miner was performing hot-work inside an excavator bucket, with all prescribed PPE, and when EE stood up, EE bumped EE's head.

Struck by falling object

When opening the door on #2 crusher tub following a plug for the second time during the shift, an approximately 8"" rock struck the top of their foot behind the safety toe. Operator reported pain and redness in the area, administered first aid. Operator received x-rays without broken bones and was removed from work for rest.

Over-exertion in lifting objects

The operator was tasked with shoveling material from a 36 inch deep sump pit of material into a skidsteer bucket so the pump could be set. The operator described the shoulder that was sore and wouldn't go away. The operator went to multiple providers which resulted in removal from work. The operator did not report any injury until 2/26/2024 for this work.

Operator was driving plastic friction pin for screen panel with dead blow hammer. While swinging the hammer, the hammer head struck the operator's left thumb causing a fracture.

The operator misjudged foot placement while ascending the access ladder to the Pit Truck, they rolled their left ankle. First-aid was first applied and then they sought professional medical attention, revealing a sprain. Operator was removed from work for two days. Operator is both near-sighted and far-sighted but was not wearing bi-focal safety glasses.

While moving a full drum of oil, it was caught on an empty drum behind it. While trying to reposition the empty drum, the full drum shifted and landed on EE's left foot. Operator was x-rayed with no fracture. The foot was bruised, the physician removed operator from work for appropriate rest. Operator was still experiencing pain, has been removed from work for orthopedic follow up.

As the employee was attempting to cut pump packing, the utility knife slipped and punctured a leg. Based on the depth of the puncture wound, the employee was transported to the local health care facility for medical treatment. The employee received three stitches to close the wound. The employee was released for full duty without restrictions.

While repairing the bucket connecting rod for the dipper arm cylinder, the operator was aligning the bucket connecting rod with a screwdriver to install a keeper bolt. The screwdriver broke under the leverage pressure causing a piece of the screwdriver to strike the operator in the safety glasses. Physician prescribed eye drops to keep the eye clean.

The operator was walking in through a personnel door, the wind shifted and slammed the door closed suddenly on the right middle finger causing a fracture to the end of the finger.

The employee was removing a pump casing to perform preventative maintenance. When the employee removed the casing, EE's hand was inside the pump. The pump shifted and EE pinched EE's right index finger between the pump case and impeller. This resulted in a break at the tip of EE's right finger and a laceration requiring several stitches. The employee was wearing gloves.

Struck against a moving object

Employee was moving a 55 gal drum off a spill control pallet on a pallet jack. During the move, the pallet jack moved and the drum tipped forward; the employee did not engage the brake. The employee subsequently strained his neck and shoulder. Employee had neck surgery 2 yrs ago. As a precaution he was taken to hospital; a CAT Scan revealed no injury other than a strain.

Struck by falling object

Employee was moving a part (excavator bucket pin) onto a handcart when the box in which the part was contained in, failed, and fell onto the employee's top left foot. There were no signs of bruising or swelling and the employee returned to work. On 9/25/14, the employee felt discomfort and sought medical attention. An X-Ray revealed a fracture in the foot.

Employee was using a sledge hammer to knock frozen feed out of a shoot. His hand slipped off the sledge hammer and he caught his finger between the sledge hammer handle and the shoot. He fractured the tip of right index finger.

Employee was performing repair work, including welding on a piece of equipment. While performing the work, a piece of metal fell behind the employee's safety glasses and became lodged in his eye.

Operator stumbled while walking across the floor casuing him to fall and hurt his leg.

Operator's were attemting to start up a rock drill in cold weather. The employee who was injured grabbed the rotating drill steel. The gloves the employee was wearing were caught on the steel and twisted around the employees hand. The top of the right thumb was amputated.

Operator was cutting paper bags open when one of the bags slipped. The operator grabbed the bag. The knife he was using was caught between the bag and his hand. He received a cut to the palm of his hand and was treated with stitches.
