Miner was performing hot-work inside an excavator bucket, with all prescribed PPE, and when EE stood up, EE bumped EE's head.

When opening the door on #2 crusher tub following a plug for the second time during the shift, an approximately 8"" rock struck the top of their foot behind the safety toe. Operator reported pain and redness in the area, administered first aid. Operator received x-rays without broken bones and was removed from work for rest.

The operator was tasked with shoveling material from a 36 inch deep sump pit of material into a skidsteer bucket so the pump could be set. The operator described the shoulder that was sore and wouldn't go away. The operator went to multiple providers which resulted in removal from work. The operator did not report any injury until 2/26/2024 for this work.

Operator was driving plastic friction pin for screen panel with dead blow hammer. While swinging the hammer, the hammer head struck the operator's left thumb causing a fracture.

The operator misjudged foot placement while ascending the access ladder to the Pit Truck, they rolled their left ankle. First-aid was first applied and then they sought professional medical attention, revealing a sprain. Operator was removed from work for two days. Operator is both near-sighted and far-sighted but was not wearing bi-focal safety glasses.

While moving a full drum of oil, it was caught on an empty drum behind it. While trying to reposition the empty drum, the full drum shifted and landed on EE's left foot. Operator was x-rayed with no fracture. The foot was bruised, the physician removed operator from work for appropriate rest. Operator was still experiencing pain, has been removed from work for orthopedic follow up.

As the employee was attempting to cut pump packing, the utility knife slipped and punctured a leg. Based on the depth of the puncture wound, the employee was transported to the local health care facility for medical treatment. The employee received three stitches to close the wound. The employee was released for full duty without restrictions.

While repairing the bucket connecting rod for the dipper arm cylinder, the operator was aligning the bucket connecting rod with a screwdriver to install a keeper bolt. The screwdriver broke under the leverage pressure causing a piece of the screwdriver to strike the operator in the safety glasses. Physician prescribed eye drops to keep the eye clean.

The operator was walking in through a personnel door, the wind shifted and slammed the door closed suddenly on the right middle finger causing a fracture to the end of the finger.

The employee was removing a pump casing to perform preventative maintenance. When the employee removed the casing, EE's hand was inside the pump. The pump shifted and EE pinched EE's right index finger between the pump case and impeller. This resulted in a break at the tip of EE's right finger and a laceration requiring several stitches. The employee was wearing gloves.

Employee was moving a 55 gal drum off a spill control pallet on a pallet jack. During the move, the pallet jack moved and the drum tipped forward; the employee did not engage the brake. The employee subsequently strained his neck and shoulder. Employee had neck surgery 2 yrs ago. As a precaution he was taken to hospital; a CAT Scan revealed no injury other than a strain.

Employee was moving a part (excavator bucket pin) onto a handcart when the box in which the part was contained in, failed, and fell onto the employee's top left foot. There were no signs of bruising or swelling and the employee returned to work. On 9/25/14, the employee felt discomfort and sought medical attention. An X-Ray revealed a fracture in the foot.

Employee pinched finger between the feed belt skirt and conveyor idler when trying to release the skirt.

Employee was wearing boots with holes in them and walked through water on the floor that contained caustic.

Employee hit his hand on the platform railing while charging the ball mill.

Employee's hearing test results show that he has sustained a standard threshold shift in his hearing.

Employee reported having traumatic bursitis of the left knee from crawling across the lower deck of the 6 X 20 to remove screens.

Employee reportedly strained his right shoulder and neck when he quickly turned his head to back up the bobcat.

Employee was using a sledge hammer to knock frozen feed out of a shoot. His hand slipped off the sledge hammer and he caught his finger between the sledge hammer handle and the shoot. He fractured the tip of right index finger.

Employee was performing repair work, including welding on a piece of equipment. While performing the work, a piece of metal fell behind the employee's safety glasses and became lodged in his eye.

Operator stumbled while walking across the floor casuing him to fall and hurt his leg.

Operator's were attemting to start up a rock drill in cold weather. The employee who was injured grabbed the rotating drill steel. The gloves the employee was wearing were caught on the steel and twisted around the employees hand. The top of the right thumb was amputated.

Operator was cutting paper bags open when one of the bags slipped. The operator grabbed the bag. The knife he was using was caught between the bag and his hand. He received a cut to the palm of his hand and was treated with stitches.

Employee's hand was caught in hose material and twisted while attempting to manipulate the hose to start slurry flow.

Employee was using a hammer to install liners on a ball mill. The motion of the hammer and the tight space caused a strain his back.

The operator was removing barrel covers from 55 gallon barrels and strained his wrist.

Operator cut his leg with his knife sutures were administered.

Slipped on loose rock and scraped hand and jammed wrist while attempting to stop the fall. Tetanus shot was administered and wrist was braced.

While performing maintaince on a chain saw, EE contacted the chain and although saw was not running, he sliced finger on chain. Medical attention required: butterfly stitches.

While EE was working on a ladder, the ladder slid. EE reached to maintain stability and in doing so, contacted a sharp object, lacerating palm of right hand.

Employee slipped when exiting of a large haul truck,using the ladder attached to the truck. Employee fell backward into the bumper of the parked haul truck.

WHILE LOADING PVC PIPE ON A TRUCK, EE STEPPED ON A PIECE OF PVC PIPE. THE PIPE ROLLED CAUSING THE EE TO FALL ONTO THE LOADING DOCK RAILING. EE LANDED ON HIS RIBS CAUSING FRACTURES TO THE RIBS AND INTERNAL BRUISING.

WHILE HANDLING MATERIALS ( STEEL ) FOR ELEVATOR REPAIRS STEEL SLID WHILE EE WAS SHIFTING THE WEIGHT. PINCHING THUMB BETWEEN STEEL & RAILING.

ROUTINE MAINTANENCE ON DRILL, JAMMED FINGER BETWEEN WRENCH & DRILL BOOM.

THE DRILLER HAD COMPLETED DRILLING A HOLE AND WAS REMOVING THE DRILL STEEL FROM THE HOLE. HE WAS USING HIS ROD CHANGER TO PLACE A STEEL BAR BACK INTO THE RACK AD WHILE HE WAS DOING THIS THE RE MAINING STEEL SLID THRU THE CENTRALIZER AND FELLBACK DOWN THE HOLE. THE DRILLER TRIED TO RETRIEVE THE STEEL WITH ANOTHER STEEL ROD BUT THAT ONE ALSO FELL DOWN INTO THE HOLE. HE THEN TOOK A SH

SLIPPED AND FELL ON SNOW COVERED GROUND CAUSING INJURY TO RIGHT KNEE.

ROUTINE CLEAN UP DUTIES, SHOVELING.

EE TRIPPED OVER A PIECE OF RUBBER HOSE IN AN EFFORT TO RESTRAIN THE FALL, HE STRAINED HIS BACK.

EE SLIPPED ON STEP WHILE CLIMBING INTO TRUCK CAB AND FELL AGAINST THE SIDE OF THE TRUCK.

EMPLOYEE FELL FROM A 6' VERTICAL LADDER WHILE TRYING TO CLIMB THE LADDER HOLDING ONTO A GREASE GUN. EMPLOYEE LANDED ON HIS BACK.

EMPLOYEE WAS SITTING IN A PIT TRUCK WAITING TO DUMP WHEN A SMALL PIT SHOT WAS DETONATED APPROX 300 YDS FROM THE TRUCK. EE REPORTED PAIN AND RINGING IN RIGHT EAR AS A RESULT OF BEING IN THE DIRECT LINE OF THE BLAST NOISE.

EE WAS SPRAYED IN THE FACE AND EYES WITH PROCESSWATER AS A RESULT OF A PUMP DISCHARGE HOSE RUPTURE.

EE SLIPPED & FELL APPROX 10' WHILE DESCENDING A FIXED, VERTICAL LADDER.

EE WAS USING IMPROPER TOOLS TO STRAIGHTEN A CRUSHED BOBCAT EXHAUST TAIL PIPE. HE WAS USING A LARGE ADJUSTABLE WRENCH, & WRENCH SLIPPED, CAUSING HIM TO CUT HIS INDEX FINGER ON HIS RIGHT HAND. H E WAS WEARING LEATHER GLOVES AT TIME OF ACCIDENT. EE WENT TO THE DOCTOR & RECEIVED 7 SUTURES. HE WILL RETURN IN 2 DAYS TO CHECK FOR INFECTION.

EE REPORTED ARM PAIN AFTER USING A LARGE HAND-HELD GRINDER.

EMPLOYEE DROPPED A 4"" HOSE NIPPLE ON HIS THUMB. LACERATIONS TO THUMB REQUIRED 2 SUTURES.

WHILE CLEANING AROUND A BALL MILL EMPLOYEE TRIPPED ON CONCRETE STEP AND FELL ON HIS RIGHT SHOULDER.

WHILE TAKING TEST SAMPLES LIQUID CAUSTIC DRIPPEDON EMPLOYEE'S LEG CAUSING A BURN.

EE WAS ATTEMPTING TO MOVE FROM ONE ELEVATED PLATFORM TO ANOTHER WHEN HE SLIPPED AND FELL ABOUT 7FEET TO A STEEL GRATED DECK.

EE SLIPPED ON A PIECE OF PIPE LAYING ON THE FLOOR AND SPRAINED HIS RIGHT WRIST WHEN HE ATTEMPTED TO STOP HIMSELF FROM FALLING.

EMPLOYEE SLIPPED ON ICE AT THE BOTTOM OF THE STAIRS IN THE MILL. HE FELL DOWN 3 STAIRS AND HURT HIS BACK. HE WAS PUT OUT OF WORK UNTIL THE 16TH OF FEB. THE INJURY WAS DESCRIBED AS A CONTUSION.

EMPLOYEE CUT FINGER ON RIGHT HAND WHEN PLOW TRUCK HE WAS DRIVING TIPPED ON ITS SIDE.

EMPLOYEE WAS WORKING IN A MOATED AREA WHEN CAUSTIC LIQUID SOLUTION ENTERED HIS BOOT CAUSING A BURN TO BIG TOE ON RIGHT FOOT.

EMPLOYEE SLIPPED ON ICE AND FELL ON HIS RIGHT WRIST.

EE WAS CLEANING OUT A DIKED OVERFLOW AREA WHEN CAUSTIC SOLUTION ENTERED HIS SHOE CAUSING A BURN TO HIS FOOT.

EMPLOYEE WAS OPERATING AN OVERHEAD HOIST WHEN A CHAIN WRAPPED AROUND HIS ARM AND CAUSED HIM TO PULL A MUSCLE IN HIS ARM/SHOULDER.

EE WAS HOLDING A PIECE OF STEEL WHILE ANOTHER EE WAS GRINDING IT AND A PIECE OF STEEL HIT THE EE IN THE EYE(RIGHT EYE)

WHILE ATTEMPTING TO MOVE A 4"" PLASTIC PIPELINE THE EE SLIPPED ON ICE & MUD FALLING TO THE GROUND. INJURING HIS BACK, NECK & SHOULDER

EE WAS STRUCK IN THE LEG AND KNOCKED TO THE GROUND WHEN A PRESSURIZED PLASTIC TAILINGS LINE BLEW APART.
