Replacement of shaker screen on the top deck of the wash plant. A torch was used to heat the bolts on the screen. It was found that two of the bolts could be removed with an impact gun. While torch cutting the first remaining bolt, it broke free and struck EE in the forehead under the brim of EE's hardhat.

Employee was bent at the waist cleaning material away from a conveyor transfer point when a rock hit employee in the back of the head.

Accident victim was operating 329 Excavator loading out overburden from an exposed face. The material under the right side track shifted and caused the excavator to slide down the slope landing about 15 LF below.

EE was walking down a small surge pile when EE slipped on a loosened rock. EE reached out with the right hand to catch self and as a result broke thumb, sprained wrist and injured other fingers.
