Southern Minnesota Construction Company, Inc. · Fall onto or against objects

Driver climbed onto rear end of belly dump trailer, while walking on platform on left hand side of trailer, he tripped over the crossover bar and fell face first into an opening on the trailer frame. (This opening is manufacturer made to allow access to mechanical parts.) Driver suffered multiple contusions to back, chest and neck.
