Center Mine Coal

Employee and Co-worker were down Slope 3 attempting to set a suction hose into a sump. The hole in the ice was not very large and both miners had to hold the pipe above their shoulders to get the end into the sump. EE was behind Co-worker when they were handling the poly pipe and stated after they had the pipe set that EE slipped a bit and felt something in EE's back.

Employee tried lifting a 4" poly pipe that was plugged with mud to try and clear the pipe. When the employee dropped the pipe, they felt something was wrong with their lower back.

EE was cleaning a scraper tire fender. EE thinks that is when the left knee was injured. EE had a limp at the end of shift.

IE was using a wrench to remove an oil filter off of a 24M during regular PM. EE was wearing rubber gloves when their finger got pinched between the wrench and a piece of metal. Injury required stitches

EE was trying to free a stuck dump rope, was using prybar and slipped and hit EE in the mouth.

EE was moving dirt with a D10 dozer when the dozer blade struck a rock and abruptly stopped the machine. The EE reported soreness to the neck and shoulder area and sought medical attention off site.

At approximately 5:30 AM. Miner parked in the employee parking lot before work shift started. They plugged in their personal vehicle also lifting their windshield wipers to prevent freezing. They stepped on an uneven surface, approximately ¼ inch or less in height of the paved parking lot, rolled their left ankle resulted in a sprain.

Employee was installing an I-beam to tie the revolving frame to the tub base and was using a hammer to tap the I-beam into place. EE struck middle finger of left hand with the hammer and fractured the tip of finger.

A mechanic was removing a park brake from a 993K loader. The park brake weighs approx. 300lbs. and shifted as EE was positioning for removal. The EE pinched their RT middle finger tip, resulting in sutures to close the wound.

Lift of structural beam being made. Miner was inside safe zone upon commencement. Load began to pivot as it came up. Miner placed self in pinch point in an attempt to stop collision between load and separate piece. Load impacted miner and piece at same time.

On 07/30/2021 a contractor did not report home & sheriff was notified. On 07/31/2021 the contractor was found unresponsive by county deputy. An ambulance and coroner were summoned & the contractor was pronounced deceased on site at 5:45am.

T142N-R84W, Sec. 22 SW Thunderstorm 5.5 Miles NW of the office. Burning bales, 'lightning' no threat to miners approx. 1.2 miles away, monitored. 4:24 PM Safety was called, MSHA guidance needed non-threat bale fire. 4:32 PM called 'voicemail' 4:34 PM WY Supervisor notified, informed us to call One Call hotline number anyway, just to keep them inform, out next morning

Miner was walking on Dragline Bench to Pickup for shift change. High Wind Event 30 to 40 mph gusts. Got a piece of dirt in Right Eye. Miner thought EE cleared it. Next morning dirt needed to be removed and treated with Tobradex Opthalmic.

Miner (welder) was air arcing on a dragline bucket in the shop, after, when miner was repositioning the torch they bumped it. The hot stinger tip struck left arm and burned through welder coat, resulting in a third degree burn approx. 1" No Restrictions, No Lost Work Days, Treated with Silvadene Prescription.

Miner received MRI 10/22/2020 to assess knee pain, was asked about daily activities Dr. said it could be a workman's comp issue Possible cause 207 loader's impeller clutch. Miner informed Mine operator on 10/28/2020. No operators had brought concerns forward no past history of events. determined surgery was needed. No LTA until 1/13/2020. Date & cause uncertain.

Miner along with 3 others were changing 736 dragline bucket. While removing the last trunnion link pin. EE struck the pin, not hitting it hard. A piece of metal ½"W x 1/4"H broke away from the Wilcox hammer, striking the miner in the left side of belly. Leaving laceration approximately 1/2" across. Surgery was required to remove the piece of metal. Resulting in lost work days.

2/25/2020 Miner was hammering 6" pipe apart and pulling and felt something in EE's rt. elbow. It was determined to be an unspecified strain on 2/28/2020 by Dr. EE was treated with Ice and OTC meds on. 6/10/2020 went back continuing problems it was determined with to be a torn muscle at would require surgery on 6/23/2020. The miner worked EE's duties up until 6/23/2020.

After a heavy three day rain & blizzard event maintenance was removing snow from parking lot. The miner was asked to move EE's car in order to better clear all the areas. The miner slipped on EE's prosthetic leg which resulting in a fall on ice striking EE's head and received a concussion and abrasion on the back of EE's head.

Miner was removing a wedge from dragline socket struck it with 20lb hammer. Piece of steel chipped off and embedded in lower lip was removed. No sutures needed Tetanus shot was administered.

Miner was cleaning grease with pre-solve on dragline walk. It was hot outside and they were sweating they took their glasses off to wipe sweat away from the Rt. eye. Miner forgot they had pre-solve on their glove, getting sweat and chemical in an eye. Antibiotic applied.

Miner was raising a loaded bucket of dirt while inching forward, front right tire sank into ground. The operator quickly lowered the bucket to avoid tipping and when bucket struck ground, the machine rocked the operator forward, causing the operator to strike the windshield with head. Operator was not wearing seatbelt at time of event as per federal and company regulations.

Miner was having problems with rt. elbow, appears to be from a previous surgery non-work related. On July 3rd the mine was informed by WSI the inflammation in the elbow was in a different location where the surgery took place year ago. This appears to have been cause, while pushing on a dragline cable during the re-socketing process in May. Currently on restricted duty

The miner was exiting cab of pickup 6. While turning left the miner's clothes snagged the driver seat while stepping out, in doing so miner twisted left ankle. Miner walked off and the next morning left ankle swollen. Miner informed supervisor at 4:30 AM on 4/25/2019. Resulting in a sprain.

Miner was starting shift and traveling to work area on the dragline bench with company pickup. EE slid into a parked grader with tail lights on, on a well-lit bench. Miner struck EE's face on steering wheel required 2 sutures on in chin one in lip. Miner was permanently suspended due to violating policies.

Miner was stripping 15kV 350MCM cable with a utility knife, ee slipped with left hand while stripping the insulation and received a laceration left 1st index finger, and 5 sutures were needed.

Moving fender from Kress 363 and had dirt fall off and must have got a piece in the eye on 09/27/2018. The miner tried to wash it out. The eye was still irritated on 09/28/2018 dirt piece need to be removed by Ophthalmologist.

Contractor janitor was retrieving an aerosol paint can from a cabinet and removing a wood lath from behind the cabinet when the cabinet and stand tipped over due to damaged leg striking the janitor in the back, causing a bruise to mid and lower back. Prescribed a days dose of Acetaminophen/Hydrocodone

Miner received a laceration between on the left hand between the thumb and pointer finger. The miner was attempting to pry a socket of a impact wrench an slipped. resulting in three stitches.

Utility miner stepped over some 6" pipe in the field and slipped on some ice twisting left knee. EE was placed on restricted duty.

Mechanic was pulling an Air hose walking backwards, 11/16/17. Mechanic tripped and fell on their right shoulder got back up and went back to work. Over the weekend the other shoulder started to hurt and they reported it on 11/21/2017 employee went to see the doctor. (strain)

Mechanic was removing a guard between the transfer case and engine on a kress coal truck. They removed 1/2 half of the guard, while handing it to the other mechanic they brushed against the other half embedding a piece of metal in the left elbow 1/2 inch with of human hair, one stich required for removal.

The miner was pushing a 6 inch pipe and stepped across a small berm onto uneven ground and rolled right ankle causing a sprain.

May 27th 3:11PM D11 426 caught fire. The miner was pushing and notice smoke come from front for the dozer while backing, ee stopped to investigate ee smelled smoke and found fire below the dozer on the Rt. side near the cab. Fire suppression and fire extinguishers were unable to extinguish the fire. Fire Dept used a foaming unit to extinguish the fire No injuries.

Apx:1:45pm noise heard from Mainshop from wash-bay Supervisor walked over to wash-bay building to confirm, at 1:54am walked over to safety department inform Safety of Hotsy chimney was blown off. I had left and was 6 miles away. Back approx 2:08pm confirmed shut down of propane, no damage to building or injuries 2:24pm called MSHA. Appears to be failed coil.

The Miner Mechanic was loading a traffic cone onto holder rod on the back of a Terex water truck and dropped the cone on finger creating a pinch point to the gloved left index finger. It created laceration. Two stitches and tetanus given.

Supervisors weekend pre-shift inspection, noticed a light plant on fire. 7 on mines on site Nearest dragline was.93 miles away and 8 miles away for help. Safety assisted over the phone. There was no other potential hazards due to location. Fire was put out with 150lbs and re-started then buried with dirt. Safety notified MSHA after knowing we had control of the fire.

The D10T Dozer operator was getting out the dozer man basket and went to remove a tiling spade from the basket, The spade was in a slot that holds the spade in place. EE reached over left side and grab the shovel. The shovel got stuck and EE pulled it again and felt something pop in lower back.

The miner stepped off the lowboy trailer and when EE got to the ground EE slipped and twisted EE's left ankle. This resulted in a sprain of unspecified ligament of the left ankle, EE was placed on restricted duty.

Miner was changing dozer tilt hose on 9/9/2015, when he felt some sort of dirt particle fall into his left eye. He tried flushing his eye afterwards. The eye was still irritated the next day and was brought to the clinic. The particle was removed and he was prescribed some Tobradex for preventative measures.

Dragline operator and oiler were putting a timber back in place under the drag cable. Oiler was using a pry bar on a timber The timber slid on the pry bar and pinched the top of the 4th little finger midpoint between the top of the finger and 1st knuckle on the right hand of the operator. Approximately 3/16"of the finger was amputated mid point of the nail.

361 experienced sudden failure on truck. Operator parked & engaged the suppression system, exited cab with hand held. Rear engine was on fire approx.60'from operators cab, moved to a safe area. No water used in-order to control leaks. Failure: drive-train area. The shaft & yoke come apart and destroyed guards, battery cables and hydraulic lines, ignition caused by battery cable.

Dozer Operator was stuck. Cat 426 was operator went assist in removing the extraction cable from Dozer 425. 530 scraper was connected Dozer 425. Operator of 425 went to help disconnect cable. 426 operator got between the cable and the dozer blade. Scraper operator 530 started to drive away. Miscommunication between operator and Supervisor.

EE was removing bearing of swing shaft cut bearing with torch, hit bearing with hammer. Sliver of steel flew off striking him in left thigh penetrating jeans and coveralls. He was taken to clinic. He was sent back to work no Meds/Restrictions/removal of piece. 11/15/13 piece of steel moved and was removed by clinic Fri 11/15/13. Informed 11/18/13 back to work No LTA/Restrictions.

A welder felt an irritation in his eye. He reported it to his supervisor and was taken to an optometrist for evaluation. A piece of metal was removed from his cornea.

Employee was welding, grinding. He was wearing sealed eye wear and a face shield when the wind picked up and debris got into his eye. He was seen at the Eye clinic and a rust ring was removed and antibiotics given.

Employee had been welding, cutting, grinding for the previous shift and the current shift. He felt an irritation in his eye. He had been wearing full PPE. He was taken to an optometrist and nothing was removed from his eye, however there was a small rust ring. He was given a prescription strength eye drop and returned to work full duty.

Miner was standing on an extension ladder his feet were on 5th rung approximately 5 to 6' off the ground. He was welding just above the ladder on the back of the kress truck. The ladder slipped out from under him. He sprained his Rt. wrist trying to catch him self. Miner stated he failed to tie off the ladder or use the bed platform. Miner was put on restricted duty.

During concrete pour line reducer was plugged. When unplugging reducer clamp was removed. After removal reducer fell causing safety whip to tighten and slide up handle. Slid across fingertip and remove 1/2" of left hand ring finger.

10/17/11 Dozer D11T 426 had trouble with the man lift it was latch in a up position and tagged out. On 10/19/11 The Mechanic & Operator worked on the man lift to repair a Bad Oil Reservoir The operator relied the latch pin and for the mechanic. The man lift dropped suddenly striking the mechanic in the left shoulder. Leaving a deep tissue bruise in the upper left arm.

Miner Welder was Air Arching both his welding helmet and safety glasses were being used, a gust of wind came up and the miner felt he got something in his Rt.Eye. He tried to clear it with the eye wash station. Later the miner went to Dr.s office to have dirt removed.

Unsecured 4' piece of angle iron fell on top of right hand as employee was backing away from work area on hands and knees.

Kress driving west, pickup was headed east the intersection headed south, speeds reduced due to blowing snow. Kress operator failed to fully yield. The contractor did not yield to mobile equipment, the pickup struck the rear duel. Called for assistance and the contractor got out of his pickup and waited for the supervisor. Contractor received a bruise from his seatbelt.

7:30am supervisor was moving miner from the T-Dz to a RT-Dz. Miner couldn't stand or bend. Asked if he hit anything or fell, he didn't know what happen. Noted, during 1st-aid, he was using a back brace, He was checked by a MD said he was having spasms. I believed it was caused by a pre-existing condition. 08/30/2010 the miner claimed a LTA.

Mechanic was working on the A/C on a Motor Grader 16M he was he was stepping down of the machine and twisted his left knee. The mechanic continued to work for an addtional 30 min before he noticed any pain in his left knee. He notified his was supervisor and was taken to the doctor office and was told he had a sprained left knee.

The Miner left a note on his time card at the end of his shift 2/26/2010 hurt right rib laying on his rt side working on a motor no medical treatment 3/1/2010 The Miner said he was ok and incident report was filled then. 3/4/2010 he claimed re-injury himself after he coughed. 3/5/2010 he saw the Dr. he return to the next work day and put on lite duty for the week.

The Dragline Mechanic was disposing of a Metal race bearing approximately 15lbs dropping it 8" into a scrap metal bin. The Dragline oiler walked by the bin at the same time. A sliver of steal flew out striking the Dragline Oiler in the Rt. Elblow. The sliver puntured the skin and had to be treated with medication to prevent infection.

Mechanic was working in a access hole on a D11-R dozer repairing an implement pump. He was in awkward position loosening a bolt. He pulled a muscle in his left shoulder.

The DL operator, was removing frozen material on the DL shoe. He used a weed burner to heat the metal. He placed it in an access hole, then placed the cover back leaving hole partially open. Then operator strated digging. The oiler was asked to check on the weed burner. The operator heard an explosion. He found his oiler lying on the top of the shoe & started emergency care.

The utility crew was moving a section of 6" discharge hose. The hose was covered with a large amount of snow. They hooked a pickup up to pull it out from the deep snow cover. When they pulled the section out, a section of 6" plastic pipe was connected to it came out. This struck just below his Rt knee, restricting his activities for three days.

The injuries occurred when the scraper operator struck a 1'-10" drainage ditch bouncing the back of the scraper. The operator was able to complete the shift that day. The next day the operator had bruised rt. and lt elbows and soreness in the lower back and hips.

Employee was working on a cat engine with four 6 ton jackstands. The employee pulled himself up from the hoses on the top of the engine. One of the stands came loose striking the employee. Injury - contusion on right foot and left thumb and scrape on shoulder. New engine stands were built and corrections made.

Two elec were cleaning collector rings in the dragline. Vacuum in the work area stirred up dust and they went to the shop to get respirators; they started coughing and could not get it under control. Both were taken to the clinic, tested and treated. Investigation showed the vacuum's motor stirred up the dust with just a filter on the vacuum; no bag corrections were made.

Two elec were cleaning collector rings in the dragline. Vacuum in the work area stirred up dust and they went to the shop to get respirators; they started coughing and could not get it under control. Both were taken to the clinic, tested and treated. Investigation showed the vacuum's motor stirred up the dust with just a filter on the vacuum; no bag corrections were made.

Dragline operator was doing rapairs on the rigging and cleaning out some frozen dirt from the drag socket with a 2' pry bar. He was pushing at it to loosen the frozen dirt. The bar glanced off, striking him behind his steel toe boot on the left foot. Injury was a laceration behind the small toe, recieved 5 sutures.

Welder was removing a wheel studs on a Coal Truck. After he was done welding a nut on one of the studs the welder raised his welding helmet. A piece of hot slag popped and flew behind his safety glasses where it lodged in his left eye. The piece had to be removed.

During an electrical inspection on the dragline, employee stepped on access cover that was not properly closed. He received a laceration to his lower RT leg, five sutures required.

Welder was starting up air arcing after a short break. He turned the air back on. The air blew some debris around getting a small piece of metal behind his safety glasses and into his right eye. The metal piece was removed and he was back to work his next shift.

Employee was standing in a 19" high stack of ripper shanks to be repaired, using remote control for a overhead crane. He was stepping clear and miss-stepped falling to the ground. This resulted in back strain, bruising and small cuts to his right forearm. He struck a bearing cooler with his forearm. Resulting in four lost days.

Worker was driving a lowboy truck with a dozer loaded. The driver's side inside duel on the trailer had side wall fail on the tire. The air pressure broke the windows in the cab behind the driver. The tempered glass broke into small pieces, causing small cuts. One on the back of the neck, 2 stitches, one to the rt side of the face, 2 stitches, & rt middle finger 6 stitches.

Employee was helping to replace a swing transmission, it was hanging crooked. They hooked a come-along to it to help level it as they lowered the transmission. The come-along swung the transmission to the side pinching the employees thumb between the shaft and a bolt. Employee continued to work, blood was drained the next day from thumb nail bed.

Employee was removing a small section of hydrulic hose from a storage reel brand "19mm 3/4" LN USA SEA100r12 4000PSI MSHA IC-84/12 H" The section of hose recoiled off the reel striking the employee in scalp causing laceration needing 8 stiches

Employee, a Mechanic, was heating a wear sleeve on a D11R. He had a heating torch in one hand & was going to reposition a second torch with the other hand. He didn't get secure grip on the torch and slipped with it. He then tried to grab for it with his Rt. Hand and in the process received a 1st & 2nd Degree burn to his Rt. wrist, approx. 1.25 inches across.

Employee called me at Home at 10:00am Aug 12, 05 he said he had strained his mid back on 2nd shift Aug 11,05 about 7:00pm ripping coal. He told his supervisors. EE felt he should see his Chiropractor, call made. Appointment set Aug 15, 05. EE worked 2nd shift the 12th and continued his normal duties. I checked on him 4:00pm and asked how he was feeling he said fine but stiff.

EE stepped over a beam in the walkway; used to slide a shaft out. He stepped off the bottom step and caught his foot on the chain, his foot rolled it to the inside, He thought he had just twisted it. Sat 7:30am He called the Safety Dir and told him about it, bruising and swelling. Told him to see a Dr. fracture to Rt.fibula, Dr visit Mon soft boot and limit movement.

Employee was using a 5/8" tugger wire rope connected to a 2" nylon sling that was chocked to the boom raising cable. The nylon sling received a cut from one of the drum clamp bolts causing the sling to tear in two. The tugger rope recoiled backwards with the socket end striking the employee on his hard hat.

The ee was climbing up and down off the boom foot and sections to use an optic scope for leveling. the ee began to complain of pain in his groin area.

EE was operating a motor grader blading subsoil when he struck a rock and jarred his back aggravating a pre-existing condition. He reported this to his supervisor and continued to work up until 8-18-04. At that time medical steps were taken to correct the problem.

EMPLOYEE LOST HIS BALANCE WHILE PICKING UP BACKUP STRIPS FOR WELDERS, FALLING FORWARD ONTO HIS CHEST. HIS LEFT LOWER ARM FELL ACROSS PLYWOOD COVERING A CABLE ACCESS HOLE PUSHING THE WOOD OVER. HIS LOWER LEFT ARM FELL INTO THE HOLE. NO OTHER CUTS OR BRUISES.

EMPLOYEE WAS WELDING VERT WALL. WHILE USING THE 5" GRINDER TO PREP THE WALL FOR WELDING THE GRINDER HUNG UP IN THE WALL ROOT OPENING AND KICKED BACK. EMPLOYEE RECEIVED LACERATIONS TO HIS NOSE WHICH REQUIRED SUTURES.

EE WAS CARRYING CARDBOARD BUCKETS OF GEAR OIL FROM A CLEANUP OF SPILLED OIL FROM INSIDE THE DRAGLINE TO A DISPOSAL BARREL OUTSIDE. EE STATES HER FOOT SLIPPED ON THE TOP STEP OF A 3 STEP STAIRW AY & SPRAINED HER LEFT ANKLE. EE STATES IT WAS RAINING & THE BOTTOM OF HER BOOTS MAY HAVE BEEN WET CAUSING HER TO SLIP. SHE WAS WEARING RUBBER OVER SHOES AT THE TIME.

EMPLOYEE WAS USING AN AIR IMPACT WRENCH TO TIGHTEN A NUT ON A HYDRAULIC CYLINDER. THE VIBRATION CAUSED THE CYLINDER TO MOVE AND IT FELL DOWN AND PINCHED THE TIP OF HIS THUMB.

EE WAS REMOVING A DUST PLUG FROM A COM-LOCK TYPE GREASE COUPLING TO FILL THE BULK GREASER ON A CAT 992 G LOADER. THE SPIRAL WOUND WIRE RING THROUGH THE DUST PLUG HAD BEEN DAMAGED. WHEN THE EE PUT HIS FINGER THROUGH THE RING AND PULLED TO OPEN A PROTRUDING WIRE PUNCHURED HIS FINGER THROUGH HIS LEATHER WORK GLOVE.

EE WAS DRILLING A HOLE IN A POLE FOR A CROSS ARM. THE DRILL BIT CAME THROUGH THE POLE WHEN THE TIP OF THE BIT CAUGHT A GROUND WIRE THAT WAS STAPLED TO THE POLE. THE BIT STOPPED SUDDENLY AND TH E DRILL HANDLE SPUN AROUND AND HIT THE TOP OF HIS RIGHT HAND.

EE WAS LOADING AN OIL BARREL ONTO THE DRAGLINE WITH AN OVERHEAD CRANE & REACHED OUT TO PULL THE BARREL INSIDE THE DRAGLINE FROM THE CRANE & STRAINED LOWER BACK.

EMPLOYEE HAD COMPLETED CLEANING THE REAR CAB WINDOWS AND WAS STANDING ON A PLATFORM ACROSS THE REAR OF THE MACHINE. HE MISSED STEPPED, LOST HIS BALANCE AND HIS HAND SLIPPED FROM THE GRAB RAIL CAUSING HIM TO FALL TO THE GROUND RESULTING IN BRUISED RIBS AND A SMALL FRACTURE TO THE LEFT WRIST.

THE DRIVER POSITIONED HIS TRUCK IN THE PIT TO BE LOADER, AND REACHED OVER TO THE DASH TO GET HIS LOAD COUNT REPORT AS THE LOADER WAS APPROACHINGTHE TRUCK TO DUMP THE BUCKET WITHT HE LOADER. THE OPERATOR DROPPED THE BUCKET FULL OF COAL STRIKING THE TOP OF THE BOX CAUSING THE TRUCK TO BOUNCE, RESULTING IN WHIP LASH TO THE TRUCK DRIVER.

EMPLOYEE WAS WELDING ON A RIPPER SHANK WHICH WAS UP ON A STAND. HE KNELT DOWN ON THE FLOOR TO INSPECT THE BOTTOM SIDE AND KNELT ON A SMALL ROCK OR A PIECE OF SLAG BRUISING HIS LEFT KNEE CAP.

EMPLOYEE WAS PRE-RIPPING COAL WITH A RUBBER TIRE DOZER. AS HE BACKED UP THE RIGHT REAR WHEEL DROPPED INTO A HOLE ABOUT 1 1/2 FEET DEEP WHERE A CHUNK OF COAL HAD BEEN PULLED UP AND JARRED HIS B ACK. EMPLOYEE STATED HE COULD NOT SEE THE HOLE FROM THE MACHINE.

OPERATOR WAS CLEANING RIGHT SIDE OF CAB WINDOW, UPON COMPLETION SHE LOST HER BALANCE AND HER LEFT HAND CAME IN CONTACT WITH THE EXHAUST PIPE. EMPLOYEE BURNED 4 FINGERS ON BACK OF LEFT HAND.

EMPLOYEE WAS CHANGING A LIGHT BULB ON A FRONT LIGHT ON HIS DOZER AND WHILE PUTTING THE LENS BACK ON, THE LENS BROKE IN HIS HAND AND CUT HIS FINGER. THE LENS MAY HAVE HAD A CRACK IN IT CAUSING IT TO BREAK.

EMPLOYEE PILED UP COAL WITHT HE FRONT END LOADER AND THOUGHT HE SAW SOME COAL ROCK. HE GOT OFF THE LOADER TO CHECK IT OUT, REACHED OUT TO MOVE A CHUNK AND STRAINED HIS BACK.

BELIEVE THIS TO BE A RESULT OF AN OLD INJURY REPORTED ON 2-27-80 OF A TORN LIGAMENT IN HIS RIGHT KNEE. IT IS EXPECTED TIME LOSS OF 10 DAYS AND 2-3 WEEKS OF RESTRICTED WORK FROM THIS SURGERY.

EMPLOYEE LIFTED UP ON A WATER HOSE AND FELT A PAIN IN HIS LOWER BACK. HE FINISHED HIS SHIFT AND WORKED THE SHIFT THE NEXT DAY. ON THE 12TH OF MARCH HE CALLED AND SAID HE AHD ENTERED THE HOSP ITAL FOR PAIN IN HIS LOWER BACK.

EE WAS GETTING A HELMET FROM LOWER SHELF BY WELDING TABLE AND KNELT DOWN ON RIGHT KNEE. WHEN GETTING UP HE FELT PAIN AND HIS KNEE WOULD NOT EXTEND. HAD ARTHROSCOPIC SURGERY TO REMOVE A PIECE O F TORN CARTLIAGES ON HIS KNE SO HIS KNEE WOULD STRAIGHTEN OUT.

EE ELECTED TO HAVE SURGERY ON HIS HAND ON 7-1-98, TO BYPASS AN ULNAR ARTERY ON HIS RT WRIST. APPARENTLY THIS CONDITION CAME FROM USING HIS RT AHND TO HOLD A WELDING STINGER, AND USING VIBRATOR Y TOOLS AND PUSHING WITH HIS HAND. EE WORKED AS A CONSTRUCTION WORKER-WELDER FOR ABOUT 20 YEARS BEFORE WORKING AS A WELDER AT THIS MINE.

EE WAS GRINDING ON A PIECE OF METAL WITH DISC GRIND. HE WAS USING TWO HANDS ON THE GRINDER AND IT SLIPPED FROM HIS LEFT HAND AND IT DROPPED DOWN AND THE DISC STRUCK HIS RIGHT KNEE ABOVE THE KN EE CAP, RESULTING IN FIVE 5 STITCHES.

EMPLOYEE WAS MOVING 20-50 POUND BOXES FROM SHELVES TO A HAND CART TO PUT IN STORAGE. HE STATED HE SQUATED DOWN AND LIFTED A BOX AN DFELT A DISCOMFORT IN HIS BACK. EMPLOYEE CALLED ON 12-1 AND SAID HE HAD HURT HIS BACK AND WAS TAKING CHIRO. TREATMENTS.

EE WAS HELPING TO RESOCKET DRAGLINE CABLE. WHEN A CABLE SOCKET WEDGE WAS LIFTED INTO THE CABLE SOCKET, THE WEDGE BECAME UNBALANCED, AND THE WIRE ROPE SLING SLIPPED AND PINCHED HIS FINGERS, FRA CTURING THE RIGHT RING FINGER ABOVE THE MIDDLE JOINT AND SEVERING THE RIGHT LITTLE FINGER TIP.

EMPLOYEE WAS HAULING CLAY TO FILL A SOFT SPOT IN THE ROAD. HE DROVE OVER A CHUNK OF CLAY TO BREAK IT UP FOR A MOTOR GRADER TO BLADE OUT. AS THE SCRAPER WENT OVER THE CHUNK, IT BOUNCED AND STRE SSED HIS LOWER BACK MUSCLE.

EE WAS PUTTING A PIECE OF SCRAP METAL INTO RECYCLE BIN & LET THE STEEL SLIDE THROUGH HIS GLOVED HANDS. A SHARP EDGE CUT THROUGH HIS GLOVE & CUT HIS FINGER.

EMPLOYEE WAS REPAIRING A LOAD COUNTER AND WAS CUTTING THE TAIL OFF A NYLON TIE STRAP WITH A UTILITY KNIFE. THE KNIFE EITHER SLIPPED OR CUT THROUGH STRAP AND RAN THE BLADE INTO HIS LEFT WRIST. HIS WRIST REQUIRED 3 STITCHES.

CHANGING CUTTING EDGE ON A 657E SCRAPER AND WHILE SETTING THE EDGE INTO PLACE, PINCHED FINGER BETWEEN NW AND OLD CUTTING EDGE.

EE WAS OPERATING A SCRAPER HAULING TOPSOIL AND A DOZER WAS PUSHING THE SCRAPER TO LOAD. THE SCRAPER GOT AHEAD OF THE DOZER AND THE DOZER BUMPED THE PUSH BLOCK HARD, JERKING THE SCRAPER OPERATO R, RESULTING IN A NECK INJURY.

EE REPORTED HE HAS HAD BURSITES IN THE PAST BUT NOT OF THIS SEVERITY. HE STATE HE WAS JUST DOINGA NORMAL JOB OF DOZING. EE STATED HE HAD OPERATED DOZER FOR 3 DAYS & HIS DOCTOR STATED THE CONDI TION CAME FROM OPERATING DOZER.

EE WAS FILLING IN A SLOPE WITH DIRT AND RAN OVER A 2' FROZEN CHUNK OF DIRT WITH THE LEFT FRONT TIRE OF HIS SCRAPER. THE SCRAPER TIRE SPUN OFF OF THE CHUNK AND HE STRAINED HIS BACK.

EE WAS REMOVING SIDE PANELS FRO THE MOTOR COMPARTMENT AND WAS STANDING ONTHE TIRE WHEN HE SLIPPED OFF AND FELL ABOUT 31/2 FT TO CONCRETE FLOOR. HE WAS PREPARING TO WASH THE MOTOR BEFORE MAINTE NANCE ON THE MACHINE. U

EMPLOYEE WAS UNLOADING A 20LB PROPANE TANK FROM A PICKUP AND STEPPED ON A BROKEN OFF STEEL POST UNDER THE SNOW, AND IT PUNTURED HIS FOOT. HE NEEDED 3 STITCHES. HE WAS WEARING STEEL TOE WORK BOOTS.

EMPLOYEE WAS OPERATING SCRAPER,HAULING DIRT IN A NORMAL MANNER,WITH RIGHT ARM EXTENDED OPERATING THE BOWL CONTROLS.HE FELT PAIN DEVELOP IN HIS ARM AND SHOULDER DURING THE SHIFT.DOCTOR RESTRICT ED HIM FROM OPERATING SCRAPER.HE MAY OPERATE OTHER TYPES OF EQUIPMENT AND DO OTHER WORK,AS LONG AS HE DOESN'T EXTEND HIS RIGHT ARM.

EMPLOYEE WAS MOVING DRAGLINE POWER CABLE WITH A DEUTZ-ALLIS CABLE TRACTOR WHEN THE WIND BLEW THE HOOD UP AND BACK, STRIKING THE LEFT SIDE OF THE HEAD. THE WIND WAS 25-30 MPH AND THE HOOD LATCH ES FAILED.

EMPLOYEE WAS LIFTING A FIVE GALLON CAN OF ANTI-FREEZE FROM THE GROUND TO ABOUT 2' HIGH INTO THE FUEL TRUCK AND FELT A PAIN IN HIS CHEST. CAN WEIGHT ABOUT 40-45 LBS.

EMPLOYEE WAS CLEANING DIRT FROM SHOP FLOOR WITH SKIDSTEER LOADER.HE TURNED AROUND AND STARTED FORWARD.THE LOADER LURCHED AND EMPLOYEE STRUCK RESTRAINING BAR WITH HIS CHEST.EMPLOYEE WAS WEARING A SEATBELT.

EE WAS HELPING TO LOAD TWO 1-1/2" CABLES INTO A PICKUP BOX. EE MOVED HIS HAND TO GET ANOTHER GRIP ON THE CABLE AND A BROKEN WIRE PUNCTURED HIS HAND THROUGH HIS LEATHER GLOVE.

EMPLOYEE WAS LOWERING AND GUIDING SWING SHAFT INTO PLACE, WHEN THE SHAFT CAUGHT ON SOMETHING. ITMOVED TOWARD EMPLOYEE AND HE STEPPED BACK OFF OFTHE GEAR CASE AND STRAINED LEFT ANKLE.

EE WAS PUTTING SCRAP IRON IN RECYCLE BIN AND A PIECE OF STEEL FROM A GEAR THAT WAS PUT IN THE BIN BROKE OFF AND HIT EE IN THE NOSE.

EE WAS OPERATING SCRAPER AND WHILE LOADING IN CUT AREA HE HIT A HOLE WITH THE TIRE, JOLTING THE SCRAPER.

EMPLOYEE WAS WASHING A SCRAPER WITH A HIGH PRESSURE HOT WATER WASHER & HE WAS PULLING UP EXTRA HOSE UP ON THE SCRAPER & THE SPRAY WAND PASSED OVER HIS BOOT BEHIND THE STEEL TOE & MADE 11/2IN C UT IN HIS BOOT HE STATED HE NEVER RELEASED THE TRIGGER

EMPLOYEE HAD TAKEN COAL SAMPLES FROM THE DRILL WHEN HE GOT BACK INTO THE CAB HE REACHED FOR THE DOOR HANDLE TO CLOSE THE DOOR A STRONG GUST OF WIND BLEW THE DOOR SHUT ON HIS THUMB HIS DOC STAT ED THE THUMB NAIL WILL GROW BACK IN ABOUT TWO WEEKS

EMPLOYEE HAD LAYED OUT A 4" WATER LINE AND WAS WALKING BACK TO THE PUMP PICKUP AND STEPPED INTO A DEEP RUT AND FELL DOWN. HE WAS WALKING AGAINST THE LIGHTS OF THE PICKUP AND DID NOT SEE THE RU T.

A FORKLIFT WAS GOING TO LIFT A RAIL WHICH WAS ON A 6INX6IN WOOD BLOCK THE FORKLIFT DRIVE INTO A SOFT SPOT IN THE GROUND THEN BACKED UP & REPOSITIONED HE DROVE IN & HIT THE RAIL PUSHING IT OFF THE BLOCK & ONTO THE SUPERVISORS STEEL TOED BOOTHE NEVER WENT TO A DOC UNTIL APRIL 28 & FOUND A SMALL FRACTURE

EMPLOYEE WAS FUELING A WATER TRUCK NEAR END OF SHIFT & UNHOOKED FUEL & TURNED AROUND TO HANG IT UP WHEN FOOT SLIPPED ON GOURND & DISLOCATED A RIB & NECK.

EMPLOYEE AND A MECHANIC WERE REPAIRING AN OIL LEAK UNDER A SCRAPER USING A PRY BAR TO HOLD HOSES BACK AND THE MECHANIC RELEASED THE BAR AND THE EMPLOYEE NEVER GOT A HOLD ON THE BAR AND IT STRU CK THE EMPLOYEE IN THE CHEEK AND RESULTED IN A CUT THAT NEEDED THREE STITCHES.

WAS GOING TO CHANGE OXYGEN TANKS ON THE TORCH.REMOVED CHAIN FROM ROW OF TANKS IN STORAGE RACK AND WHILE REMOVING A TANK,THE TANK FELL FORWARD,SMASHING THE LITTLE FINGER NEEDING 14 STITCHES.

EE WAS AIR ARCHING A CRACK IN A WELD AND SPARKS FELL INTO HIS SWEATSHIRT HOOD WHICH WAS OUTSIDE OF HIS WELDING LEATHER JACKET AND STARTED TO BURN THE HOOD. HE REACHED BACK WITH HIS LEFT HAND T O BEAT OUT THE FIRE AND LOST HIS WELDING GLOVE. BURNING HIS HAND SWEATSHIRT HAD PLYESTER AND COTTON IN IT.

EE WAS WALKING FROM OPERATOR S CAB AND HIS RIGHT HEAL CAUGHT ON THE EDGE OF THE TOP STEP AND HE FELL INTO THE HAND RAIL ON THE STEPS.

EMPLOYEE WAS RIPPING FROZEN GROUND WITH A D-10 CAT AND BOUNCING OVER FROZEN DIRT CHUNKS. HE STATED PAIN IN HIS BACK PRGRESSED AS THE NIGHT WENT ON. EMPLOYEE CALLED IN SICK THE NEXT MORNING. IT WAS NOT UNTIL JANUARY 24 HE CALLED AND REPORTED TO ME HE WAS GOING IN FOR BACK SURGERY MONDYA, JANUARY 27 AND STATED RUNNING CAT ON JANUARY 7 WAS THE CAUSE OF HS BACK PROBLEM. ALSO IN JAN

EE WAS LEANING OVER A CABLE SPOOL OF 2 INCH X 68 FT OF CABLE AND CUT THE STAPLE HOLDING THE CABLE END. THE CABLE UNWRAPPED AND STRUCK HIM OF HIS LEFT SIDE OF HIS HEAD ON THE SAFETY GLASSES AND BROKE THE NOSE PAD CUTTING HIS NOSE ON THE LEFTSIDE.

EMPLOYEE WAS TO FUELD A WATER PUMP. WHEN WALKING TO THE PUMP HE STEPPED INTO A DEPRESSION ON THE GROUND AND HURT HIS LEG. THIS WAS NOT REPORTED BY EMPLOYEE UNTIL 9-24-91.

EE WAS BLDG FENCE AND WAS WRAPING THE SECOND WIRE FROM THE TOP AROUND THE CORNER POST AND SCRATCHED HIS ELBOW ON THE TOP WIRE LOOP AND BARB WIRE CUT HIS ELBOW 3/4INCH LONG. HAD TO HAVE 10 STIT CHES IN LEFT ELBOW.

HE THEN PLACED HIS RIGTH HAND ON THE DOOR POST AND USED HIS OTHER HAND TO SHUT THE DOOR AND PINCHED HIS R IGHT THUMB BETWEEN THE DOOR AND POST ON THE HINDGE SIDE. HE RECEIVED TWO STITCHES IN RIGHT THUMB BY THE NAIL.

EMPLOYEE WAS PUTTING A TOOTH ON A RIPPER SHANK FROM A D-10 CAT WHICH HAD BEEN WELDED UP IN THE WELDING SHOP. THE TOOTH WOULD NOT FIT SO EMPLOYEE USED A 12 LB HAMMER AND STRUCK THE TOOTH TO DR IVE IT INTO PLACE. A CHIP OF STEEL FROM THE TOOTH HIT HIM IN THE LEG. EMPLOYEE REPORTED ON FEB. 13 HE HAD BEEN TO DOCTOR TO HAVE CHIP REMOVED.

EMPLOYEE'S MACHINE WAS IN THE SHOP BEING REPAIRED. HE WAS ASKED TO HELP MOVE A TRANSMISSION ON A WHEEL CART. HE WAS PUSHING ON THE CART WITH THREE OTHER EMPLOYEES WHEN HE FELT HIS BACK GO OUT. HE WORKED FOR ABOUT TWO HOURS MORE THEN LEFT WORK TO SEE A CHIROPRACTOR.

EMPLOYEE WAS PUSHING ON A WRENCH AND ANOTHER EMPLOYEE WAS PULLING ON THE WRENCH. THEIR HANDS SLIPPED OFF THE WRENCH AND HIT THE INJURED EMPLOYEE IN THE RIBS. THEY WRE TIGHTENING A BOLT ON THE SWING SHAFT.

EE WAS FUELING A SCRAPER WHEN HE FELT CHEST PAIN AND PAIN IN HIS ARM. HE CALLED HIS SUPERVISOR AND ASKED TO BE TAKEN HOME. SUPERVIOSOR TOOK HIM HOME AND MADE SURE THE EE S WIFE TOOK HIM TO THE DR AND GET CHECKED OVER. HE WAS PUT IN A HOSPITAL AND HAD TRIPPLE BY PASS SURGERY JUNE 25, 1990.

EMPL WAS DRIVING 120 COAL HAULER IN PIT BOTTOM & DROVE OVER COAL CHUNK ABOUT 15" IN DIAMETE WITH RIGHT FRONT TIRE OF TRUCK. HE STATED HE SAW CHUNK & TURNED TO MISS IT BUT HIT IT WITH FRONT TIR E.

EMPLOYEE WAS OUT BY THE TUB OF THE DRAGLING MOVING POWER CABLE. AS HE WAS WALKING AWAY FROM THE SHOE OF THE DRAGLINE, HE STEPPED ONTO A SMOOTH PAD OF DIRT LEFT BY A DOZER TRACK AND THE PAD OF DIRT MOVED SLIGHTLY AND HE TWISTED HIS ANKLE AS HE STEPPED ONTO IT.

EMP WAS WALKING3DOWN A SLIPED GRASS SNOW COVEREDEMBANKMENT TO CHECK A SEDIMENTATION POND. HIS RT FOOT SLIPPED AND HE FELL BACKWARDS AND CAUGHT HIMSELF WITH HIS LEFT HAND. HE WAS NOT WEARING O OVERSHOES, JUST3SMOOTH BOTTOM BOOOTS

AN EMP WAS UP O3 16 FT SCAFFOLD ATTEMPTING TO SCREW A 1X11 PIPE INTO AN AIR COUPLING 20 FT OFF THE FLOOR. THE PIPE SLIPPED FROM EMP HANDS AND FELL TO THE FLOOR AND HIT THE EMP WORKING ON THE FLOOR BELOW IN 7HE ANKLE. THE NEXT MORNING THE INJ EMP SAW A DR AND HE WAS ADVISED TO STAY OFF HIS FOOT AND ELEVATE FOR TWO DAYS

EMPLOYEE PUT ON3THE CLIMBING HOOKS TO CLIMB A WOOD POWER CABLE ARCH POLE TO PUT THE STEEL CABLE BACK INTO THE SHEAVE. ON HIS WAY DOWN THE POLE ABOUT 15 FT FROM THE BOTTOM ONE OF HIS SPIKES LET GO FROM THE PO5E AND HE SLID IN HIS SAFETY BERLXXX BELT. THE POLE WAS IN GOOD CONDITION AND ALSO WERE HIS SPIKES.

EE WAS TRYING TO FREE A PULLEY THAT WAS JAMMED BETWEEN A MOTOR MOUNT & CRANKSHAFT DAMPER. THE PULLEY CAME FREE & HE JAMMED HIS THUMB AGAINST A BRACE.

EMPL PUSH-PULLING WITH ANOTHER SCRAPER.FRONT SCRAPER WENT DOWN IN SAND AND REAR SCRAPER CLIMBED UP ON PUSH BLOCK.WHEN FRONT SCRAPER LOADED AND PULLED AWAY,REAR SCRAPER DROPPED DOWN AND BOUNCED CAUSING INJURY.WAS USING SEAT BELT.

EE WAS LIFTING ROLLERS FROM A CAT ON TO PALLETS.

EMPL REMOVING BOLTS FROM TRACK PAD.BOLT WOULDNT LOOSEN SO HE TOOK SLEDGE HAMMER AND HIT BOLT HEAD TO LOOSEN IT,WHEN CHIP FROM HAMMER FLEW OFF AND STRUCK HIM IN THE NECK.

LIFTING POWER CABLE TO BE LOADED ONTO REEL TRUCK WHEN HIS FOOT SLIPPED IN THE SNOW-COVERED GROUND CAUSING HIM TO FALL TO GROUND.HE WAS WEARING GOOD OVERSHOES.

EMPLOYEE WAS SITTING AND GOT UP FROM A STOOL TO GET MORE WELDING RODS AND BUMPED HIS ELBOW ON A DRAGLINE BUCKET HE WAS WELDING ON.

EE WAS REMOVING A BEARING RACE. HE CUT THE RACE WITH A TORCH THEN HE USED A HAMMER TO REMOVE THE RACE FROM THE SHAFT & A SMALL PIECE OF STEEL CAME OFF THE RACE & LODGED IN H IS FINGER.

EMPLOYEE WAS PUTTING 1/4" STEEL SHEET INTO THE STORAGE RACK AND THE CLAMP WEDGED. HE THEN TOOK A PRY BAR TO FREE THE CLAMP. THE SHEET TIPPED UPAND STRUCK HIS ARM.

2 EMPL LIFTING ROLLER OUT FROM REVOLVING FRAME ON DRAGLINE WITH BAR THROUGH OLLER WITH MAN ON EACH SIDE.IT SLIPPED OUT OF HAND AND ROLLER SLIDE STRIKING TOP OF HAND CUTTING IT.PALM OF HAND HIT CABLE HOLDER ON TUB AND CUT PALM REQ STITCHES ON TOP AND BOTTOM OF LEFT HAND.

EMPLOYEE WAS DISMOUNTING A MOTOR GRADER AT THE END OF THE SHIFT AND MISSED THE SECOND STEP OF THREE AND FELL TO THE GROUND AND LANDED ON MOWBOARD OF THE GRADER. EMPLOYEE STATED HE HAD HOLD OF HAND RAILS BUT MISJUDGED HIS STEP.

EE WAS UNTANGLING A ROPE FORM AROUND A DISCHARGE HOSE ON A WATER PUMP. HE WAS STANING IN ABOUT 1" OF WATER ON TH E CLAY & HIS FOOT SLIPPED & FELL ON HIS RT KNEE.

EMPLOYEE HAD TAKEN OUT A PIN FROM THE POWER STEP TO HAVE A KEEPER WELDED ON THE PIN. UPON INSTALLING THE ARM THE STEP MOVED AND THE EMPLOYEE LOST HIS BALANCE AND FELL 8\ TO THE GROUND. THIS S TEP WAS ON A CAT DOZER D9L.

EMPLOYEE WAS REMOVING A SHAFT FROM A GEAR WITH A PUNCH AND A HAMMER, WHEN A CHIP OF METAL FROM THE PUNCH LODGED IN HIS WRIST.

EMP WAS DRIVING TRACTOR UP SLOPE ON FOURTH ATTEMPT IT SPUN OUT DUE TO ICE AND SNOW EMP PROCEDED TO BACK DOWN IN REVERSE TRACTOR SLID INTO AND OVER THE BERM AND PROCEDED DOWN THE SLOPING SIDE EMPLOYEE JUMPED FROM THE TRACTOR BEFORE IT OVERTURNED

EE WAS TIGHTENING BOLTS ON A SWING BOX ON THE 736 DRAGLINE WITH AIR WRENCH WHEN AIR HOSE CAME OFF THE COUPLER & STRUCK EE.

EMPLOYEE WAS REPAIRING ON THE DRAGLINE ROLLERS AND JUMPED UP ONTO THE TUB AND STRUCK HIS KNEE ON A SHAFT STICKING OUT FROM A ROLLER.
