Employee 1 was operating a pickup truck that was being backed up to hook up to a pump so it could be relocated. Affected employee was guiding the truck to align the pintle hitch of the pump. As the trucks bumper and the pintle hitch made contact, employee pinched a finger as they were trying to align the pin that didn't fully seat itself.

EE was replacing a lip shroud on an Esco dragline bucket. While removing the wedge that locks the lip shroud to the bucket, the lip shroud fell (10 inches) onto the EE's feet. The bucket was located in the welding shop. EE was wearing steel toed boots.

Employee and Co-worker were down Slope 3 attempting to set a suction hose into a sump. The hole in the ice was not very large and both miners had to hold the pipe above their shoulders to get the end into the sump. EE was behind Co-worker when they were handling the poly pipe and stated after they had the pipe set that EE slipped a bit and felt something in EE's back.

EE was moving dirt with a D10 dozer when the dozer blade struck a rock and abruptly stopped the machine. The EE reported soreness to the neck and shoulder area and sought medical attention off site.

At approximately 5:30 AM. Miner parked in the employee parking lot before work shift started. They plugged in their personal vehicle also lifting their windshield wipers to prevent freezing. They stepped on an uneven surface, approximately ¼ inch or less in height of the paved parking lot, rolled their left ankle resulted in a sprain.

A mechanic was removing a park brake from a 993K loader. The park brake weighs approx. 300lbs. and shifted as EE was positioning for removal. The EE pinched their RT middle finger tip, resulting in sutures to close the wound.

On 07/30/2021 a contractor did not report home & sheriff was notified. On 07/31/2021 the contractor was found unresponsive by county deputy. An ambulance and coroner were summoned & the contractor was pronounced deceased on site at 5:45am.

Miner (welder) was air arcing on a dragline bucket in the shop, after, when miner was repositioning the torch they bumped it. The hot stinger tip struck left arm and burned through welder coat, resulting in a third degree burn approx. 1" No Restrictions, No Lost Work Days, Treated with Silvadene Prescription.

Miner received MRI 10/22/2020 to assess knee pain, was asked about daily activities Dr. said it could be a workman's comp issue Possible cause 207 loader's impeller clutch. Miner informed Mine operator on 10/28/2020. No operators had brought concerns forward no past history of events. determined surgery was needed. No LTA until 1/13/2020. Date & cause uncertain.

Miner along with 3 others were changing 736 dragline bucket. While removing the last trunnion link pin. EE struck the pin, not hitting it hard. A piece of metal ½"W x 1/4"H broke away from the Wilcox hammer, striking the miner in the left side of belly. Leaving laceration approximately 1/2" across. Surgery was required to remove the piece of metal. Resulting in lost work days.

2/25/2020 Miner was hammering 6" pipe apart and pulling and felt something in EE's rt. elbow. It was determined to be an unspecified strain on 2/28/2020 by Dr. EE was treated with Ice and OTC meds on. 6/10/2020 went back continuing problems it was determined with to be a torn muscle at would require surgery on 6/23/2020. The miner worked EE's duties up until 6/23/2020.

After a heavy three day rain & blizzard event maintenance was removing snow from parking lot. The miner was asked to move EE's car in order to better clear all the areas. The miner slipped on EE's prosthetic leg which resulting in a fall on ice striking EE's head and received a concussion and abrasion on the back of EE's head.

Miner was cleaning grease with pre-solve on dragline walk. It was hot outside and they were sweating they took their glasses off to wipe sweat away from the Rt. eye. Miner forgot they had pre-solve on their glove, getting sweat and chemical in an eye. Antibiotic applied.

Miner was raising a loaded bucket of dirt while inching forward, front right tire sank into ground. The operator quickly lowered the bucket to avoid tipping and when bucket struck ground, the machine rocked the operator forward, causing the operator to strike the windshield with head. Operator was not wearing seatbelt at time of event as per federal and company regulations.

Miner was having problems with rt. elbow, appears to be from a previous surgery non-work related. On July 3rd the mine was informed by WSI the inflammation in the elbow was in a different location where the surgery took place year ago. This appears to have been cause, while pushing on a dragline cable during the re-socketing process in May. Currently on restricted duty

The miner was exiting cab of pickup 6. While turning left the miner's clothes snagged the driver seat while stepping out, in doing so miner twisted left ankle. Miner walked off and the next morning left ankle swollen. Miner informed supervisor at 4:30 AM on 4/25/2019. Resulting in a sprain.

Miner was starting shift and traveling to work area on the dragline bench with company pickup. EE slid into a parked grader with tail lights on, on a well-lit bench. Miner struck EE's face on steering wheel required 2 sutures on in chin one in lip. Miner was permanently suspended due to violating policies.

Mechanic was pulling an Air hose walking backwards, 11/16/17. Mechanic tripped and fell on their right shoulder got back up and went back to work. Over the weekend the other shoulder started to hurt and they reported it on 11/21/2017 employee went to see the doctor. (strain)

Mechanic was removing a guard between the transfer case and engine on a kress coal truck. They removed 1/2 half of the guard, while handing it to the other mechanic they brushed against the other half embedding a piece of metal in the left elbow 1/2 inch with of human hair, one stich required for removal.

May 27th 3:11PM D11 426 caught fire. The miner was pushing and notice smoke come from front for the dozer while backing, ee stopped to investigate ee smelled smoke and found fire below the dozer on the Rt. side near the cab. Fire suppression and fire extinguishers were unable to extinguish the fire. Fire Dept used a foaming unit to extinguish the fire No injuries.

Apx:1:45pm noise heard from Mainshop from wash-bay Supervisor walked over to wash-bay building to confirm, at 1:54am walked over to safety department inform Safety of Hotsy chimney was blown off. I had left and was 6 miles away. Back approx 2:08pm confirmed shut down of propane, no damage to building or injuries 2:24pm called MSHA. Appears to be failed coil.

The Miner Mechanic was loading a traffic cone onto holder rod on the back of a Terex water truck and dropped the cone on finger creating a pinch point to the gloved left index finger. It created laceration. Two stitches and tetanus given.

Supervisors weekend pre-shift inspection, noticed a light plant on fire. 7 on mines on site Nearest dragline was.93 miles away and 8 miles away for help. Safety assisted over the phone. There was no other potential hazards due to location. Fire was put out with 150lbs and re-started then buried with dirt. Safety notified MSHA after knowing we had control of the fire.

Miner was changing dozer tilt hose on 9/9/2015, when he felt some sort of dirt particle fall into his left eye. He tried flushing his eye afterwards. The eye was still irritated the next day and was brought to the clinic. The particle was removed and he was prescribed some Tobradex for preventative measures.

Dragline operator and oiler were putting a timber back in place under the drag cable. Oiler was using a pry bar on a timber The timber slid on the pry bar and pinched the top of the 4th little finger midpoint between the top of the finger and 1st knuckle on the right hand of the operator. Approximately 3/16"of the finger was amputated mid point of the nail.

Employee was helping to replace a swing transmission, it was hanging crooked. They hooked a come-along to it to help level it as they lowered the transmission.
