Fall from machine

EE was climbing down from EE's dozer and EE's foot slipped off of the foot rail. EE fell 2-3"" feet to the ground and rolled EE's ankle when EE hit the ground. EE said EE's leg was sore but didn't require immediate medical attention. The next day EE went to the doctor and they said EE strained EE's knee EE was told to ice and wrap EE's knee, but was able to return to full duty

Flash burns (electric)

Miners were switching the belt motor from reverse to forward on the Thunderbird under belt conveyor. Two miners were inside Switch Van One, one miner was switching the wires to reverse the motor. The other miner was acting as an observer and communication relay. While doing so there was an arc flash inside of the electrical box.

Struck against stationary object

Miner reached into an opening between two conveyors. EE's glove contacted a tail pulley and subsequently lacerated the back of EE's hand. EE received sutures and returned to work. An investigation was conducted, full report will be attached or mailed.

Two persons working on Canica. 5# sledge pounding, fragment lodged in the calf of 2nd miner. Treated at Urgent Care. No follow up is expected.

Operating excavator, started feeling pain in back and numbness in leg & foot.

Fall from machine

Operator was climbing into excavator. Hand slipped and lost grip, fell backwards onto their back on the ground. Has previous back surgery, now has low back pain. We called their surgeon who said go to Emergency department.

Fabricating a hopper, miner was working on a ladder. Ladder slipped and miner fell approx 8 feet to the ground.

Routine changing of screen in screen plant. Miner noticed discomfort in the back. Chose not to seek any treatment until 6/6/2013.

Lifting end of pipe, felt pain but did not seek medical attention. Treatment is now progressing to medical treatment, so now filing report.

Struck by flying object

Person was using a hand-held grinder in the shop on Tuesday. He was using eye protection. There was no immediate thought that he had received a foreign body, but the next day the eye became irritated. On Thursday he was seen by a doctor who removed a foreign body from the eye. There has been no on-going treatment needed and no lasting effects to the eye.

EE WAS WELDING AND GRINDING ON GEAR CASE. A PIECE OF STEEL HIT EE IN RIGHT EYE. IT WENT UNDER HIS WELDING HOOD AND GLASSES.

WAS WASHING OUT TRUCK BOX WHEN EYES WERE SPLASHED WITH MUD & WATER. HAD BEEN HAULING MUD OUT OF WASH PLANT POND. EYES WERE RED & IRRITATED. LATER REALIZED CHEMICAL IS USED FOR WASH PLANT.

WHILE ASSISTING LIFTING & SECURING A CUTTING EDGE ON AN EXCAVATOR HE STRAINED HIMSELF.
