WORKER GRABBED WRENCH TO LOOSEN SCREEN BOLT. IN THE PROCESS OF POSITIONING SELF ON GUARDED CATWALK, WORKER PUSHED HIMSELF BETWEEN MID-RAIL AND THE BOARD. WORKER FELL TO GROUND - A DISTANCE O F APPROX 9.5 FEET.
