Miners were attempting to lift a rock out of jaw crusher with a chain attached to 980 loader bucket, when operator was told to lower bucket, employee placed his hand on the jaw hopper frame, under bucket and pinched his fingers between bucket and hopper frame, resulting in two smashed fingers.

The lid on the VSI was opened at the end of the shift to do maintenance. It was left open overnight to finish the next day. When they started changing lid liners, they didn't know the Hyd pressure had bled off but the lid was stuck in the raised position. EE stood on the lid & it dropped apprx 2 ft causing him to loose balance & fall off. He was not wearing fall protection.
