Konen Rock Products Inc · Struck by falling object

Worker was doing routine maintenance on skid steer bucket (welding). Chose to lift bucket and weld on it at an unsafe height while it was supported only by a chain instead of setting it on a block or welding table. Bucket fell on worker's toe while worker was welding.

Konen Rock Products Inc · Caught in, under or between a moving and a stationary object

Employee was filling a tote bin with Nitrogen that would be used for loading blast holes. The 2000 lb bag of nitrogen was lifted over the bin and the miner cut a hole in the bottom of the bag to allow nitrogen to flow into the bin. As the material began to flow out of the bag, it shifted and pinned the worker to the top edge of the tote, causing spinal and other injuries.
