Worker was operating an impact gun when the sockets came off, hitting EE in the chin. EE has a laceration on the chin that EE sought medical attention for. EE received 8 stitches and was released that same evening.
Worker was climbing down access ladder, slipped and fell landing on his right leg, breaking his femur.
While attempting to secure a 1 gallon can with Bungee while standing on operator access, bungee broke and operator lost balance falling & breaking ankles.
WHILE WALKING TOWARD STOCKPILE TO RETRIEVE SMALLPIECE OF SCREEN, TRIPPED OVER ROCK, PUT HAND OUT TO BREAK FALL, HAND HIT ROCK, CUT HAND & BRUISED SHOULDER. TREATMENT WAS 8 STITCHES FOR CUT & SHOULDER IS STIFF & SORE.
LOOSE CLOTHING WAS CAUGHT BY CONVEYOR ROLL AND HAND WAS PULLED IN BETWEEN ROLL AND CONVEYOR BELT.
