While adjusting a hydraulic cylinder with a portable pump a quick coupling fitting malfunctioned leading the crew to believe the pressure was bleed off when it was not. while removing the quick coupling from the cylinder it flew off and hit the miner in the arm causing tore muscle and damaged artery. The miner is expected to miss 4 to 6 weeks of work.

WHILE USING AN IMPACT WRENCH WORKER CAUGHT FINGER TIP BETWEEN SOCKET AND TURNING NUT. LEFT INDEX FINGER TIP WAS CUT AND BONE BROKEN.
