IW was using hammer to loosen wedge blocks. A shard of metal came loose and stuck in their arm, causing a laceration.

Retrieving a tool from truck bed. 5"" drill bit slipped out of hand landed on left foot just above steel toe tip of boots.

EE was sitting on a little stool leaning forward to change brakes and hub assembly of a truck. When EE sat up EE's lower back spasmed.

EE was working on the tail pulley adjuster to provide slack for the conveyor. While removing the plug from the adjuster, the plug struck EE's left index finger, palm and thumb.

EE was changing the wheel on a hand grinder. EE accidentally depressed the trigger lock and trigger. The grinder then jolted from EE's hand and cut the left ring finger.

Employee was climbing up the steps to the Haul truck while holding on to the hand rail. Employee heard a pop in right middle finger.

Throughout the day when working ee felt calf muscle giving out. When ee stepped into truck to leave at end of work day, ee heard a pop and felt intense pain.

Employee was changing screens and the screen fell, causing a laceration to left middle finger.

Employee got the right foot stuck in the mud. When trying to free their foot from the mud they then twisted their knee.

Employee slipped on snow-covered ice during heavy snowfall in the operator's parking area. Employee fell backward onto left shoulder.

EE was using a ratchet and it slipped off the bolt and struck EE above the right eye.

He was pulling a roll a way chute closed. He strained his upper back.

Lifting motor with chain hoist. The chain became disconnected and pinched left index finger causing laceration and fractures.

Employee was welding a loader bucket and took his gloves off to use a sledge hammer and accidently grabbed the hot spot on the loader bucket. He received a second degree burn on his right hand palm.

Hose with emulsion blew out of bore hole and sprayed employee in eyes and face.

Employee was using a wrench to install a roller on a crushing plant. When he pulled on the wrench he had a pain in his chest. He then was short of breath. The evening of 4/16/13 he was admitted to the hospital with a collapsed lung.

Employee was cutting a rubber hose with a utility knife and the knife slipped and cut his left palm.

Employee was hitting a scraper blade and rust flew into his right eye.

Employee was using a cutting torch to cut steel and a spark went into his ear.

Employee was walking from one crusher to another and slipped and fell on a ramp, straining his back.

Employee has been a crushing plant operator for several years and now has noticed induced hearing loss.

Employee was hammering a bucket tooth and a piece of metal broke from the tooth and lodged in the surface of his abdomen.

Employee was striking a chisel with a hammer and a small piece of metal broke off and lodged in his left hand.

Employee was removing a flip gate at the wet screen of the crushing plant and strained his lower back

Employee was setting a pin press tool into its case and the press rolled onto his left pointer finger and punctured it.

The injured employee and another employee were carrying a piece of angle iron. One employee dropped his end of the iron causing it to cut the other employee's hand. The laceration required stitches.

Employee was exiting his truck and slipped on ice. He hit his left forearm on the running board of the truck.

Employee was clearing off a conveyor belt and slipped on ice, spraining left knee.

Employee was removing a tooth from a loader bucket with a hammer. A piece of metal broke off and hit him in the face.

EMPLOYEE WAS CLOSING A LARGE GATE WHEN HE STEPPED ON A ROCK AND TRIPPED, CAUSING A FRACTURE IN HIS LEFT FOOT.

EMPLOYEE WAS HELPING PULL A 400 LB. PART (REDUCER) OFF DELIVERY VAN AND STRAINED HIS BACK.

EMPLOYEE FELL.

EE WAS CHECKING THE OIL LEVEL IN THE CONVERTER OF A DRAGLINE. HE PLACED HIS LEFT HAND INTO THE ENGINE FAN.

EMPLOYEE WAS LIFTING A WINDOW ON A DRAGLINE, LEFT FOREARM BECAME SORE.

EMPLOYEE WAS HOLDING A BOLT WHILE REMOVING THE NUT WITH AN AIR GUN, BOLT CUT FINGER.

EMPLOYEE REPORTED NUMBNESS IN HIS RIGHT HAND. HE WAS SEEN BY OUR COMPANY PHYSICIAN AND RETURNED TO WORK. HE IS NOW SCHEDULED FOR SURGERY 6/22/01

EMPLOYEE WAS GOING DOWN AN EMBANKMENT AND SLIPPED.

WORKING FROM LADDER, WAS TRYING TO REMOVE A CLEVIS AND THE LADDER MOVED AND FOOT SLIPPED (SINCE IT WAS RAINING) FELL BACKWARDS, HITTING HIS BACK OFF A PIECE OF ANGLE IRON.
