IW was using hammer to loosen wedge blocks. A shard of metal came loose and stuck in their arm, causing a laceration.

Retrieving a tool from truck bed. 5"" drill bit slipped out of hand landed on left foot just above steel toe tip of boots.

EE was sitting on a little stool leaning forward to change brakes and hub assembly of a truck. When EE sat up EE's lower back spasmed.

EE was working on the tail pulley adjuster to provide slack for the conveyor. While removing the plug from the adjuster, the plug struck EE's left index finger, palm and thumb.

EE was changing the wheel on a hand grinder. EE accidentally depressed the trigger lock and trigger. The grinder then jolted from EE's hand and cut the left ring finger.

Employee was climbing up the steps to the Haul truck while holding on to the hand rail. Employee heard a pop in right middle finger.

Throughout the day when working ee felt calf muscle giving out. When ee stepped into truck to leave at end of work day, ee heard a pop and felt intense pain.

Employee was changing screens and the screen fell, causing a laceration to left middle finger.

Employee got the right foot stuck in the mud. When trying to free their foot from the mud they then twisted their knee.

Employee slipped on snow-covered ice during heavy snowfall in the operator's parking area. Employee fell backward onto left shoulder.

Employee was welding a loader bucket and took his gloves off to use a sledge hammer and accidently grabbed the hot spot on the loader bucket. He received a second degree burn on his right hand palm.

Hose with emulsion blew out of bore hole and sprayed employee in eyes and face.

Employee was using a wrench to install a roller on a crushing plant. When he pulled on the wrench he had a pain in his chest. He then was short of breath. The evening of 4/16/13 he was admitted to the hospital with a collapsed lung.
