Struck by falling object

The contract employee was helping to install a counterweight on Gradall XL5110 Scaler. As the weight was lifted into place, one of the connector links of the chain used for lifting broke, causing one end of the weight to strike the ground. This in turn broke the other end, which allowed the weight to strike the employee as it fell.

OPERATOR HAS A BACK ACHE FROM THE SEAT CUSHIONS BEING WORN OUT. OPERATOR CANNOT WORK BECAUSE OF THE BACKACHE. THE CUSHIONS HAVE SINCE BEEN REPLACED.

EMPLOYEE SLIPPED ON THE ICE. WHEN THEY FELL, THEY STUCK A HAND OUT TO CATCH THEMSELVES. WHEN THEY DID THAT, THEY DISLOCATED THEIR SHOULDER.

EMPLOYEE WAS REMOVING THE SWING PUMP FROM THE MACHINE. THE PUMP WAS STUCK BUT THEN BROKE FREE UNEXPECTEDLY. WHEN IT BROKE FREE, IT BENT EMPLOYEE'S ARM IN AN AWKWARD POSITION

OPERATOR STEPPED OUT OF THE CAB ONTO THE CATWALK TO WIPE EE'S WINDOWS. WHEN EE LEANED OVER, EE'S HARD HAT FELL OFF. EE LEANED OVER MORE TO PICK UP EE'S HARD HAT AND WHEN EE ROSE BACK UP, EE HIT EE'S HEAD ON A ROOF PROTECTOR.

Over-exertion in lifting objects

Employee was changing a flat tire on the service truck. EE lifted the tire without assistance and then was diagnosed with a ventral hernia.

EMPLOYEE WAS CLIMBING INTO THE SERVICE TRUCK, EMPLOYEE SAID EMPLOYEE CAUGHT THE SOLE OF EMPLOYEE'S BOOT AND TWISTED KNEE

Employee walked to the rear of the haul truck to check lights for the pre-trip inspection. EE twisted ankle on the berm behind the truck. Upon examination, EE has edema and bruising in the heel. Employee placed in a CAM boot, modified duty until MRI is taken.

EMPLOYEE WAS WALKING FROM THE WASTE OIL TANK TO THE SERVICE TRUCK. EE SLIPPED AND FELL AND TRIED TO CATCH SELF. WHEN EE TRIED TO CATCH SELF, EE TORE THEIR BICEP.

EMPLOYEE WAS CLEANING THE CAB OF EMPLOYEE'S TRUCK. EMPLOYEE CAUGHT A FINGER ON SOMETHING UNDERNEATH THE SEAT, CAUSING A LACERATION.

EMPLOYEE WAS OPERATING A HAUL TRUCK INSIDE OF THE MINE. THE TRUCK WAS LOADED GOING UP HILL WHEN FOR SOME UNKNOWN REASON THE TRUCK DID A ""WHEELIE"" AND ENDED UP SITTING ON IT'S TAILGATE WITH THE CAB OFF OF THE GROUND. THE OPERATOR SPRAINED THEIR LOWER LUMBAR.

Employee was loading ANFO into the hole for the shot. When EE pulled the hose out, it slipped out of EE's hand, striking EE in the shoulder.

Employee jumped off of a service truck to the ground instead of using the steps on the truck.

Employee was walking in the mine. Employee slipped and fell, when employee fell employee put arm out to catch self and jammed shoulder. Employee initially did not seek medical attention but after recently having an MRI, it shows damage that will have to be operated on.

The employee was reinstalling a bed liner into a pickup truck. Employee was using the portable crane on the service truck. When they lifted up on the crane, the snatch block came unattached from the bed liner. The snatch block swung and hit them in the head.

Employee got a foot stuck in mud, they tripped and fell forward hitting head on a rock. Hard hat prevented any lacerations but they strained their neck muscles upon impact.

Employee exited the water truck and was walking toward the drill. As ee was walking ee twisted ankle on a rock or uneven ground.

Struck by flying object

Employee was doing maintenance, reached down to pick up an air tool and the air hose came loose from the tool, striking the employee in the face and the eye, knocking off hard hat and glasses.

Employee was manually closing a door on a chute. The door did not close completely and kicked back. The handle of the door pinched the employee's finger and fractured it.

Employee was assisting another employee who was removing and replacing a belt idler. The belt was not operating and was locked out at the time of repairs. While the two employees were lifting the idler, it slipped and the idler frame fell on the employee's finger.

A rock fell onto the machine that the employee was operating, the sudden impact shook the operator in the cab. The employee finished his shift that day which ended at 3:00 PM, but called the following day and reported back pain.

Employee was lifting up a cribbing block and felt pain in his back.

Flash burns (welding)

Employee was working in a confined space with another employee who was welding. The injured employee suffered arc flash as a result.

Employee was opening up the entrance gates to the quarry, slipped on the ice and fell, hitting his head causing a laceration.

Employee was using a cable type come-along. Employee got his finger caught between the cable and the frame of the conveyor that he was working on.

Employee took the radiator cap off of the machine while it was still under pressure. To escape the spray he jumped backwards, hitting his arm and back on the machine carriage.

Employee was replacing the battery in a Euclid rock truck. Employee suffered back strain lifting the battery in and out of the truck.

Employee was backing a loaded haul truck on a downhill grade in the underground mine. He lost control of the truck and ran backwards into a pillar.

Employee was dragging a water hose, he slipped and fell, injuring his shoulder.

Operator got out of his machine and picked up a broken shank and tip. He hoisted the broken piece onto the catwalk of the machine. Employee stated he felt pain in his groin at this time.

One employee handed a piece of steel plate to another employee. The 2nd employee did not have a good grip on it and it fell to the ground with his finger underneath of it.

Employee was riding in a truck. When he went to step down out of the truck he missed the grab rail with his right hand, but was able to catch himself with his left hand. When he caught himself he thinks he pulled a muscle or something.

Employee was picking up material from the ground, there was a snake underneath the material, which bit him on the finger.

The employee (a blaster trainee) was assisting the licensed PA blaster in setting off a shot. After the shot went off (approximately 8-10 seconds) the employee went into the line of the shot and was struck in the shin by a piece of flying debris, requiring him to get 6 stitches in his leg.

Employee was helping to assemble framework for a new conveyor. He was attempting to align a bolt into it's corresponding hole, he got his finger caught between the threads of the bolt and the framework.

EMPLOYEE STATED THAT HE HURT HIS BACK HELPING TO LIFT SCREEN PANELS THAT WERE BEING INSTALLED IN THE SCREEN DECK. EMPLOYEE DID NOT REPORT PROBLEM TO MANAGEMENT UNTIL 5/3/04.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was operating the salt spreader truck. He got out of the vehicle and later was having symptoms of a heart attack. The ambulance took him to the hospital and he was diagnosed with carbon monoxide poisoning. It was later determined that the truck had a faulty exhaust system and fumes were entering the cab.

THE EE AND THE FOREMAN WERE REMOVING A BROKEN DRILL STEEL FROM THE HOLE THAT WAS DRILLED, THE BROKEN STEEL SLIPPED FROM THE EE'S HANDS AND CAUGHT HIS LEFT THUMB BETWEEN THE DRILL STEEL AND THE SASH ON THE DRILL.

EE WAS ATTEMPTING TO REMOVE AN OBSTRUCTION IN THE TRANSFER BIN TO THE CONVEYOR. WHILE DOING THIS TASK, HE ENTERED THE BIN WHEN HE EXITED THE BIN HE SLIPPED AND FELL 10 TI 11 FT. TO THE GROU ND BELOW. THIS AREA HAD LOOSE SHOT ROCK (10""MINUS) ON THE GROUND WHICH HELPED INFLICT HIS INJURIES.
