WHILE ASSISTING MECHANIC IN REPAIR WORK EE SLIPPED ON RUBBER TIRE OF LOADER AND FELL AGAINST LOADER COUNTERWEIGHT. EE FINISHED WORK SHIFT ON DAY OF ACCIDENT AND ALL SCHEDULED SHIFTS UNTIL LAID OFF 1-31-03. EMPLOYER WAS NOTIFIED OF ACCIDENT 1-27-03.

EE WAS BACKING END DUMP UP A 3 TO 4% RAMP TO DUMP. WHEN EE BACKED AGAINST DIRT PILE AT THE TOP OF THE RAMP, THE TRUCK STALLED AND DRIFTED FORWARD ABOUT 100' AND LODGED AND STOPPED ON THE BERM. EE EXITED THE TRUCK ONTO THE OUTSLOPE OF THE BERM. ONCE ON THE GROUND, HE LOST HIS BALANCE AND FELL DOWN THE SLOPE AND SUFFERED MULTIPLE LACERATIONS AND BRUISES.
