As employee was lifting a drilling table, the employee turned to push the table and felt a discomfort in right knee. The employee stopped work and notified the EHS professional and manager. Employee taken to clinic for an evaluation.

Walking towards plant - slipped on ice - bruised/sprained left knee. ***Did not seek medical attention until 1/21/2013. Had surgery on 3/7/13. Returned to work on 5/6/2013.
