As employee was lifting a drilling table, the employee turned to push the table and felt a discomfort in right knee. The employee stopped work and notified the EHS professional and manager. Employee taken to clinic for an evaluation.
Walking towards plant - slipped on ice - bruised/sprained left knee. ***Did not seek medical attention until 1/21/2013. Had surgery on 3/7/13. Returned to work on 5/6/2013.
Standing on ladder thawing pipe. On way down ladder, slipped and fell to floor.
EE WAS SERVICING EQUIPMENT, TRIPED AND LANDED ON METAL DRAIN PAN. EE CUT PALM OF LEFT HAND REQUIRING 11 STITCHES.
